摘要
患者女,88岁。左手背部出现斑块结痂渐加重1年余。HIV抗体检测阴性,皮损组织真菌培养阴性,罗氏培养基(37℃,28℃)出现阳性菌落,抗酸染色阳性,以分枝杆菌共引物对临床分离株进行PCR扩增后,可见439bp清晰条带。诊断:结核分枝杆菌皮肤感染。给予利福平、氨苯砜、四环素三联药物治疗40d后停药,3个月治愈。
A 88-year-old female presented with cutaneous infection on left hand and became exacerbation gradually for 1 year. The lesion gradually developed into scabby plaque spreading to forearm for one year. The patient was negative for HIV antibody. Colonies was seen to grow on Lowenstein-Jensen（L-J） medium at 37℃ and 28℃. Acid-fast stainning was positive of bacilli. PCR with general primer of mycobacteria showed the clear band of 439 bp. The diagnosis was cutaneous infection with nontuberculous mycobacteria. The patient was treated by rifampin, tetracycline, DDS for forty days. The leison was dispeared after three months.
