经筋刺法结合穴位注射治疗中风后痉挛性瘫痪临床研究

Clinical Study on Combined with Acupoint Injection Treatment of Spastic Paralysis after Stroke by Needling Method

目的：探讨经筋刺法结合穴位注射对中风后痉挛性瘫痪的临床效果，为提高临床疗效提供参考。方法：将符合中风后痉挛性瘫痪纳入标准的60例患者随机分为两组：治疗组共30例，采用经筋刺法配合穴位注射治疗；对照组30例，采用传统针刺法治疗。治疗组取鱼际、尺泽、通里、大陵、曲泽、血海、阴陵泉、照海、三阴交、太溪等穴，实施经筋刺法。针刺结束后，选取3—4个穴位，用维生素B12注射液进行穴位注射。每日1次，10天为1个疗程。连续治疗3个疗程。运用Fugl—Meyer运动功能评分法来测定肢体运动功能；日常生活能力通过Barthel指分级法来评定；参照《神经功能缺损程度及生活状态标准》（全国第四届脑血管病学术会议制订）对患者治疗前后的神经功能缺损值、积分值进行评定。结论：经筋刺法结合穴位注射能够有效改善中风偏瘫痉挛状态患者的运动功能和日常生活能力，能够有效地减轻中风偏瘫痉挛患者的神经功能缺损程度，从而提高患者生活质量。

Objective： To study the clinical efficacy of muscle region needling combined point injection for spastic paralysis after stroke to improve the clinical efficacy of reference. Methods ：60 patients of meeting with the inclusion criteria spastic paralysis after stroke were randomly divided into two groups ： treatment group （30 cases） which used muscle region needling combined point injection to treat;control group （30 cases） which used traditional acupuncture. Acupuncture point took the treatment group of Yuji, Daling, Tongli, Chize, Quze, Xuehai, Yinlingquan, Sanyinjiao, Zhaohai, Aixi were treated by muscle region needling. After the needling,three or four points were given injection in vitamin B12, once a day orally for 10 days as a course treatment. The Ashworth rating scale was used to evaluate limb spasticity ; Fugl - Meyer score was used to assess dysfunction of stroke. The Barthel Index Table Evaluation of acupuncture was used for hemiplegic stroke patients with activities of daily living to improve the situation. Stroke Clinical Neurological Impairment Score Standard established in 1995 by China was used to observe cases of stroke in patients with neurological impairment. Conclusion ： The combination of muscle region needling and point injection can relieve spastic paralysis after stroke in patients with muscle tension and improve post - stroke spasticity and motor function in patients with actives of daily living, can effectively reduce stroke in patients with spastic paralysis degree of neurological deficit.
