摘要
目的探讨结直肠癌组织中环氧合酶-2(Cox-2)与拓扑异构酶-Ⅱ(Topo-Ⅱ)的表达变化及其对奥沙利铂化疗患者预后的影响。方法随机选取69例大肠癌患者手术切除的癌组织、癌旁组织标本各69份,另取正常大肠组织标本20份,采用免疫组化SP法检测上述标本中Cox-2、Topo-Ⅱ表达。大肠癌患者术后均采用含奥沙利铂方案化疗。采用χ2检验、Log-rank分析和Cox风险模型分析Cox-2、Topo-Ⅱ表达对患者生存期的影响。结果癌组织Cox-2表达阳性率为88.4%(61/69),明显高于癌旁和正常组织(P<0.01);Topo-Ⅱ表达阳性率59.4%(41/69),与癌旁组织(56.5%)及正常组织(55.0%)表达差异无统计学意义;癌组织中Cox-2与Topo-Ⅱ表达呈正相关(r=0.254,P<0.05),Cox-2表达、Topo-Ⅱ表达是影响患者预后的独立因素。结论大肠癌组织中Cox-2、Topo-Ⅱ均呈高表达,二者可能在肿瘤MDR发生中起重要作用;其表达水平升高是影响患者术后生存期的重要因素。
Objective To investigate the expression changes of cyclooxygenase 2 ( Cox-2), topoisomerase- Ⅱ( Topo- Ⅱ ) in the colorectal cancer tissues and its effect on prognosis of patients with colorectal cancer treated by adjuvant oxalipla- tin based chemotherapy. Methods We randomly collected carcinoma tissue and adjacent tissue specimens in 69 cases of patients with colorectal cancer who underwent surgical excision, and another 20 cases of normal colon tissue specimens were selected. The expression of Cox-2 and Topo- Ⅱ in these specimens was detected by immunohistochemical SP method, and all patients received adjuvant oxaliplatin chemotherapy. The effect of Cox-2 and Topo- Ⅱ expression on the lifetime of pa- tients was analyzed with Chi-square test, Log-rank test and Cox regression model risk. Results The positive expression rate of Cox-2 was 88.40% (61/69) in the colorectal carcinoma tissues, which was significantly higher than those in the ad- jacent and normal tissues (P 〈 0.01 ) ; the positive expression rate of Tope-Ⅱ was 59.4% (41/69) in the colorectal carci- noma tissues, 56.5% in the adjacent tissues and 55% in the normal tissues. The expression of Cox-2 was positively corre- lated with Tope-Ⅱ expression in the colorectal carcinoma tissues ( r = 0. 254, P 〈 0.05 ). COX regression model risk analy- sis showed that the Cox-2 expression and Tope-Ⅱ expression were independent tactors 'affecting the prognosis of patients. Conclusions The Cox-2 and Tope-II are both highly expressed in the colorectal carcinoma, and they may play important roles in the occurrence of MDR in tumor. Its elevated expression level is an important factor of influencing lifetime of pa- tients after operation.
