高敏心脏肌钙蛋白检验临床应用前需解决的问题

摘要
目前临床医师开始使用新一代心脏肌钙蛋白(cTn)检验界限值:99百分位数正常上限值。以前不愿使用这种界限值,是由于非急性心肌缺血性心脏疾病导致cTn的频繁升高。使用肌钙蛋白的绝对界限值,其与以前认为诊断金标准的敏感性较差的标记物(如肌酸激酶MB同工酶)相等或检验获得10%变异系数的最低值,认为可降低假阳性。但是使用99百分位数正常上限可增加急性心肌梗死和结构性心脏病的检出能力。同时这种变化不应该与更新一代检验方法的更替相混淆。
