生物型硬脑膜补片在开颅去骨瓣减压术中的应用效果

Application of artificial biological dural graft in the operations of decompressive craniotomy

目的探讨生物型脑膜补片在开颅去骨瓣减压术中的应用价值。方法将84例行开颅去骨瓣减压术患者随机分为2组，观察组42例术中采用生物型人工脑膜修补硬脑膜，对照组42例单纯行去骨瓣减压术，对比2组术后相关并发症发生情况及随访结果。结果观察组术后脑膨出、脑组织骨窗疝、脑脊液漏、癫痫及颅内感染发生率明显低于对照组。2组脑室穿通畸形、皮下或硬膜下积液发生率比较无显著性差异。术后随访10～24个月，观察组恢复良好率明显高于对照组，对照组中一重残率明显高于观察组。结论开颅去骨瓣减压术中应用生物型硬脑膜补片能降低并发症发生率，改善患者生存质量，具有临床应用价值。

Objective It is to explore the application value of artificial biological dural graft in decompressive craniotomy. Methods 84 patients undergoing decompressive craniotomy were randomly divided into 2 groups： 42 cases with artificial bio- logical dural graft as observational group, 42 cases with single decompressive eraniotomy as control group. The post-operated complications and follow-up results were compared between both groups. Results The incidence of post-operated encephalo- cele, brain tissue bone window hernia, cerebrospinal fluid, epilepsy and encephalic infection in observational group were sig- nificantly lower than that in control group. There was no significant difference in the incidences of ventricle through deformi- ties, subcutaneous or subdural effusion in both groups. The follow-up investigation lasted for 10 to 24 months, and there was a marked higher recovery rate in observational group than that in control group, especially in the moderate and severe disability rate. Conclusion The application of artificial biological dural graft in decompressive craniotomy could decreased the incidence of post-operated complications, and improved the patients＇living quality, so it has important value in clinical treatment.
