肺纤维化患者血中细胞外基质成分的变化及临床意义 被引量:5

Changes of the Blood Levels of Extracellular Matrix Components in Patients with Pulmonary Fibrosis

摘要 为探讨肺纤维化患者血清 型前胶原 (PC )、 型胶原 (C )及透明质酸 (HA)水平的变化及临床意义。采用放射免疫分析法、血气分析仪、肺功能仪分别测定 2 0例肺纤维化患者和 2 0例正常人血清 PC 、C 和 HA含量及血气、肺功能参数。结果 :肺纤维化患者血中 PC 、C 和 HA明显高于正常人 (P<0 .0 5或 <0 .0 1) ,三者之间呈明显正相关 (P<0 .0 1) ;且血清 HA、PC 、C 水平均分别与动脉血氧分压 (Pa O2 )、肺活量 (VC)、肺弥散量 (DL co)呈负相关 (P<0 .0 5或 P<0 .0 1)。结论 :肺纤维化患者血清 PC 、C 和 HA含量增加 ,并与肺功能损害程度密切相关 ,检测其水平有助于判断临床病情及预后。 This study was conducted to explore the changes of PCⅢ(type Ⅲ procollagen), CIV(type Ⅳ collagen) and HA (hyaluronan) in the serum of patients with pulmonary fibrosis. The blood levels of PCⅢ, CIV and HA in 20 patients with pulmonary fibrosis and 20 normal subjects were measured by radioimmunoassay and were analysed in relation with the artery blood gas and pulmonary function parameters. The result revealed that the blood levels of PCⅢ, CIV and HA in the patients were significantly higher than those in normal subjects, and were negatively correlated with PaO2 (oxygen pressure in artery blood),VC (vital capacity) and DLco (lung diffusing capacity). Also, there was positive correlation between PCⅢ, CIV and HA ( P <0.01). These findings suggest that the blood levels of PCⅢ, CIV and HA of patients with pulmonary fibrosis are significantly increased and significantly correlated with pulmonary function damage and that the levels of PCⅢ, CⅣ and HA in the serum might reflect the severity of the disease and play a role in estimating prognosis.
