红细胞分布宽度与扩张型心肌病患者心力衰竭的关系探讨 被引量:9

Relationship Between Red Blood Cell Distribution Width Level and Heart Failure in Dilated Cardiomyopathy Patients

摘要 目的:探讨红细胞分布宽度(RDW)与扩张型心肌病(DCM)患者心力衰竭的关系。方法:选取105例DCM患者和40例对照组患者,检测各组患者RDW水平,并评价RDW水平与DCM患者心力衰竭的关系。结果:与对照组相比,DCM患者血RDW水平明显升高(F=5.58,P<0.01)。DCM患者心功能越差,血RDW水平越高,心功能Ⅳ级患者的血RDW水平高于心功能Ⅱ级、心功能Ⅲ级的患者,差异具有统计学差异(F=3.44,P<0.05)。血RDW水平与DCM伴心力衰竭患者左心房内径(LAD)、左心室舒张末期内径(LVEDD)呈正相关(r=0.307,P<0.01;r=0.459,P<0.01)、左室射血分数(LVEF)呈负相关(r=-0.418,P<0.01)。结论:DCM患者血RDW水平升高,RDW可以对不同心力衰竭程度的DCM患者提供有价值的参考依据。 Objective: To investigate the relationship between red blood cell distribution width (RDW) and dilated cardiomyopathy (DCM) patients associated with heart failure. Methods: From Januar- y 2008 to February 2012, 105 patients with DCM and 40 control patients were enrolled in the study, the RDW was detected and the relationship between the RDW level and heart failure in DCM group was evaluated. Results: Compared with the control group, the DCM patients' blood RDW levels were significantly increased (F=5.58, P〈0.01). The worse the cardiac function of patients with DCM, the higher the level of blood RDW. The blood RDW levels of NYHA 1V heart function patients were higher than that of NYHA Ⅱ and Ⅲheart function patients (F= 3.44, P〈0.05). The blood RDW levels were also positively correlated with left atrial diameter (LAD) and left ventricular end-diastolic diameter (LVEDD) (r=0. 307, P%0.01; r=0. 459, P %0.01), and negatively correlated with left ventricular ejection fraction (LVEF) (r=-0. 418, P%0.01) in DCM patients with heart failure. Conclusion: The blood RDW level increases in DCM patients with heart failure, and its detection can provide valuable information for different degrees of heart failure in patients with DCM.
