雾化吸入抗生素治疗呼吸机相关性肺炎的现状 被引量:9

摘要 呼吸机相关性肺炎(VAP)是机械通气的常见并发症,发病率根据患者群体的不同可达9%~70%。一旦发生VAP,其病死率可高达25%~50%。VAP的治疗一直是临床医生面临的难题。多药耐药(MDR)菌感染可导致VAP的治疗难度进一步增大。该类细菌对常用抗生素不敏感,而敏感抗生素如多粘菌素类、氨基糖苷类等虽然可以起到一定的抗菌作用,但其治疗窗口较窄,易出现不良反应,从而限制了在临床的使用。局部给药在取得疗效的同时可以降低全身不良反应。大量研究证实通过雾化吸人给药治疗哮喘、慢性阻塞性肺疾病能够取得良好的疗效,同时减轻了全身不良反应。
