低管电压结合低剂量对比剂在多层螺旋CT头颅动脉血管成像中应用的可行性研究 被引量:27

摘要 目的探讨多层螺旋头颅动脉低管电压、低剂量对比剂扫描的可行性。方法(1)水模研究:采用64层分别设置80、100、120、140kV及100、200、300、400、500、600m为扫描参数,对水模中含有不同碘浓度对比剂的试管进行扫描,经分析后选择最佳信噪比(N)与对比噪声比(N)的扫描条件进行临床研究。(2)临床研究:前瞻性连续选择2012年4月至10月疑似颅内动脉瘤、动静脉畸形等疾病的患者66例行头颅检查。随机数字表法分为、2组,各33例。组(常规组)扫描参数:120kV、400m,对比剂用量1ml/kg;组(试验组)扫描参数:80kV、600m,对比剂用量0.5ml/kg。使用独立样本检验对2组的辐射剂量、N、N进行统计学分析。由2名资深影像医师以5分制对2组的图像质量进行主观评价,使用Mnn—WhineyU检验对主观评分进行统计学分析并进行一致性检验。结果、组的容积剂量指数(DIv01)分别为59.1、27.5mGy。组较组下降了53.5%。组与组的N、N分别为71.5±10.7、92.0±7.4,58.5±10.5、79.6±7.7,组较组的N、N分别提高了22.3%、26.4%,差异均有统计学意义(值分别为584.0、388.6,P值均〈0.05)。2名医师分别对、2组图像进行主观评价,结果均为组的图像质量明显高于组(Z值分别为31.2、23.4,P值均〈0.01),差异均有统计学意义。2名医师对图像的评估具有高度一致性(Kpp值为0.904,P〈0.05)。结论在头颅检查中使用低管电压结合低剂量对比剂,不但图像质量得到提高,而且辐射剂量明显降低。 Ojeiveoevluehefeiiliyoflow-doeonmediumpooolfo64-liengiogphy()ofheduinglow-ue-volgeeing.Mehod(1)Phnomudy:64-liepilepeivelye80,100,120,140kVnd100,200,300,400,500nd600mnningpmeeline.nlyiofheeignl-o-noieio(N)ndononoieio(N).(2)liniludy:opopeivelyoninuoueleixy-ixpienwihupeedinnilneuymndeiovenoumlfomionndoonfompiloOoeof2012opefomhndinpeion.hendomnumelemehodfo,wogoup,33pieninehgoup.Goup(onvenionlgoup)nningpmee:120kV,400mndmounofongen:1ml/kg;Goup(egoup)nningpmee:80kV,600m,mounofongen0.5ml/kg.Uingindependenmpleefoehgoupdiiondoe,N,Nweeiillynlyzed.ywoexpeieneddiologio5poinonwogoupofimgequliyujeiveevluion.Mnn-WhineyUewuedfoiilnlyiofujeiveevluion.eulGoupndgoupDIvolepeivelyi59.1,27.5mGy.Gouphningoup,DIvoldeeedy53.5%.,goupofheN,Nwee71.5±10.7,92.0±7.4;58.5±10.5,79.6±7.7,GouphningoupN,Nweeenhnedy22.3%,26.4%,diffeeneweeiiignifine(=584.0,388.6,P〈0.01).heujeiveimgeevluionofgoupwighehngoup(Z=31.2,23.4,P〈0.05),diffeeneweeiiignifine.wophyiinonimgeoeheonienyofunifomheigh(Kpp=0.904,P〈0.05).onelmionInhehedexminion,ueoflowuevolgeominedwihlowondoenoonlynimpoveheimgequliyulodiiondoeweeignifinlylowe.
