全脑CT灌注联合血管成像在评价短暂性脑缺血发作中的应用 被引量:13

摘要 目的探讨320排容积低剂量灌注(P)联合技术在评价短暂性脑缺血发作(I)患者脑组织微循环改变中的应用价值。方法应用320排容积,对26例单侧大脑中动脉(M)狭窄或闭塞的I患者进行低剂量全脑P成像,同时获得M责任区患侧的血流量(F)、血容量(V)、平均通过时间(M)、达峰时间(P)、延迟时间(DLY)等灌注参数和动态4D—图像。采取镜像测量技术测得M责任区健侧对应部位脑组织上述灌注指标的具体量化值,并计算不同程度M狭窄或闭塞患者患侧/健侧的半定量值;采用动态4D.图像观察血管狭窄部位、程度以及侧支循环状况;轻中度狭窄与重度狭窄或闭塞组间各参数比较采用成组设计z检验。结果(1)26例患者均成功完成全脑P及成像。(2)26例M狭窄或闭塞的I患者中,M轻中度狭窄17例(1例合并同侧大脑前动脉轻度狭窄、1例合并同侧大脑后动脉轻度狭窄),M重度狭窄或闭塞9例(2例合并同侧大脑后动脉轻度狭窄,l例合并同侧大脑前、后动脉轻度狭窄);Willi环后循环开放14例,软脑膜吻合支形成16例。(3)M供血区患侧与健侧各灌注参数差异均有统计学意义。患侧F、V均明显低于健侧,健侧M、P、DLY均明显低于患侧。M轻中度狭窄组MY、P、DLY患侧/健侧比值明显低于M重度狭窄或闭塞组,轻中度狭窄组分别为1.06±0.06、1.05±0.07、1.42±0.66,重度狭窄或闭塞组为1.12±0.09、1.10±0.04、1.98±1.21,差异均有统计学意义(值分别为-1.798、-1.583、-1.536,P值均〈0.01)。结论-站式全脑P联合成像技术在直观显示病变血管与异常灌注区的位置关系的基础上,还可反映脑组织的微循环状况,能够很好地评估单侧M狭窄或闭塞I患者的脑血氧供应和脑血流动力学。 OjeiveExploehevlueofheinpeionehniquewhihhe320-owvolumelowdoehepefuionimgingominedwihugiogphyinheemenofhngeininiueofpienwihnienihemikmioiulion.Mehodppliionofhe320-liedynmivolume,low-doewholeinPimgingof26pienwihunilelmiddleeeley(M)enoiooluioninpienwihI,oiningMeofeponiiliyofeelflow(F),eelvolume(V),menniime(M),imeopek(P)nddelyime(DLY)pefuionpmeenddynmi4D-imge.kehemiomeuemenehniqueomeuedMeofeponiiliyofheffeediniuendheonleloepondingpofheiniue'pefuionindiopeifiquniivevlue,wellvyingdegeeofMenoiooluionofheioofipilelndonlelemi-quniivevlue;Dynmi4D-imgeoevedheieofvulenoi,hedegeeofenoindheolleliulionu,ewuedfoheiinlyiineweenmildomodeeenoigoupndeveeenoiooluiongoup.eulwenyixpienuefullyompleedhewholeinPndimge.wenyixpienofMenoiooluioninpienwihI,Mmildomodeeenoie17pien(1pienofipilelneioeeleywihmildenoi,wopienofipilelpoeioeeleywihmildenoi),Meveeenoiooluionof9pien(2pienofipilelpoeioeeleywihmildenoi,Ipienofipilelneioeeleywihmildenoindpoeioeeleywihmildenoi).hepoeioiulionofwilliopened14pien,16pienoflepomeningelnomoinhfomion.heipilelF,Vweeignifinlylowehnheoulel,onlelM,P,DLYignifinlylowehnheffeedide.heioofheMwihmildomodeeenoigoupofM,FP,DLYwee(1.06±0.06),(1.05±0.07),(1.42±0.66);heioofheMwiheveeenoiooluiongoupofM,P,DLYwee(1.12±0.09),(1.10±0.04),(1.98±1.21).heioofheMwihmildomodeeenoigoupofMF,P,DLYinipilelndonlelweeignifinlylowehnheMeveeenoiooluiongoup(=-1.798,-1.583,-l.536,P〈0.01).onluionheehnologyofone-opPominedwihimgingefleiniuemioiulionwhihedonviulizehepoiionlelionhipofhevulleionwihnomlpefuion,emenofunilelMenoiooluionofeeloxygenupplyndeelloodflowmehniinpienwihI.
