肠镜喷洒并口服吡喹酮治疗血吸虫病慢性腹泻的疗效观察

Therapeutic effect observation of schistosomiasis chronic diarrhea patients are treated by spraying praziquantel solution under the colonoscope and oral praziquantel talblet

目的探讨肠镜喷洒联合口服吡喹酮治疗血吸虫病慢性腹泻的可行性和疗效。方法因慢性腹泻作纤维结肠镜检查证实为结直肠血吸虫病患者26例(实验组),行肠镜下喷洒0.2%吡喹酮溶液,然后口服吡喹酮片,60mg/(kg·d),连服3d,观察治疗效果,并与血吸虫病慢性腹泻患者单纯口服吡喹酮治疗(对照组)比较。结果实验组26例经肠镜喷洒并口服吡喹酮治疗1周后肠镜复查,结肠黏膜下虫卵均死亡,治愈率为100%,对照组治愈率为84.00%,差异有统计学意义(χ2=4.51404,P<0.05);实验组和对照组不良反应发生率分别为84.6%(22/26)和88.0%(22/25),差异无统计学意义(χ2=0.00 002,P>0.05)。结论肠镜下喷洒并口服吡喹酮治疗血吸虫病慢性腹泻疗程短,治愈率高,且无明显药物不良反应,未增加治疗费用,值得临床推广应用。

Objective To investigate the practicability and therapeutic effect of schistosomiasis chronic diarrhea patients treated by spraying praziquantel solution under the colonoscope and oral praziquantel tablet. Methods Twenty-Six chro- nic diarrhea patients （ as therapeutic group） were proved the colorectal schistosomiasis patients, and they were sprayed praziquantel solution under the fibre colonoscope, thereafter oral 60 mg/（kg · d） praziquantel tablet for serial 3 day,and compared with that of the contrast group （ these patients with schistosomiasis chronic diarrhea were treated by only oral praziquantel tablet）. Results The schistosomiasis eggs under the colorectal mucos membrane of the 26 patients （ thera- peutic group ） treated by spraying praziquantel solution and oral praziquantel tablet were died under the colonoscope after therapy one week. The patients （ therapeutic group ） were all recovered and discharged. The recovery rate of the thera- peuticgroup（100%） was good than that of the contrast group（84.0%） （X2=4.51404,P〈.05）. The side effect of the therapeutic group（84.6% ） is not significance difference to that of the contrast group（88.0 % ） （X2=. 00002, P〉0.05）. Conclusion The therapeutic effect of the schistosomiasis chronic diarrhea patients treated by spraying praziquantel solu- tion under the colonoscope and oral praziquantel tablet is good, the therapeutic recover time is short, and the therapeutic expenses and the side effect not increase. This therapeutic method is worthy of extending and applying.
