某三甲医院361例住院患者中药注射剂用药分析研究

Study and analysis of traditional Chinese medicine injections use for 361 cases of inpatients in a 3A hospital

目的通过对某三甲医院住院患者中药注射剂使用情况的调查、点评、分析,找出存在的问题,反馈给临床各科室,以进一步促进临床合理用药。方法将该院2011年3月—2012年2月间每月统计出的前20位非抗菌药物(按用药金额排序)中属中药注射剂的药物筛选出来,抽取其中列出的使用量最大的前三个科室、每科室排名前10位的住院患者病历号,共得病房处方361例,以中药注射剂临床应用指导原则、临床路径、临床诊疗指南和药品说明书等资料为点评标准对其进行初评和终评,并将结果进行统计分析。结果在抽查出的361例处方中,不合理处方为219例,占60.66%。不合理处方主要表现为:用法用量不正确、重复用药、联用不合理、有配伍禁忌等。结论该院应加强中药注射剂处方的规范管理及合理用药宣传工作,以进一步提高处方质量,保障医疗安全。

Objective Through the investigation,review and analysis of the usage of TCM injections in inpatients of a 3A hospital,we found out the problems and made feedback to each clinical department,in order to further promote rational drug use in the clinic. Meth- otis By screening out the monthly statistics of the top 20 of non-antimicrobial agents belong to TCM injections between March 2011 to February 2012（ according to the order of sum of money） ,we drew the inpatients medical records of the maximum amount of the first three departments and the top l0 ranking in every department, and collected ward prescription in 361 cases, and then we had cases the prelimi- nary and final evaluated according to commenting criteria for TCM injections clinical practice guidelines, clinical pathway, clinical guide- lines and instructions, and the results were statistically analyzed. Results In selected examination of 361 cases of prescription, unrea- sonable prescription were 219 cases accounting for 60.66%. Unreasonable prescription were shown as：incorrect usage and dosage, re- peated drug use, unreasonable combination and incompatibility, etc. Conclusion The hospital should strengthen the standard manage- ment of TCM injections prescription and the propaganda work of rational drug use, in order to further improve the quality of prescription and ensure the safety of medical treatment.
