摘要
目的观察6分钟步行试验(6MWT)评价高压氧对左室射血分数正常的心力衰竭(HFNEF)患者心功能的影响。方法随机将100例HFNEF患者随机分为两组,对照组50例应用常规抗心衰药物治疗,治疗组在常规药物治疗基础上加用高压氧治疗,应用彩色多普勒超声检测治疗前及治疗后3个月左心室舒张功能指标,按不同心功能分级行6MWT,研究两者的相关性。结果两组治疗后,左室舒张功能指标[等容舒张时间(IVRT)、舒张早期二尖瓣血流速度E峰(E)、舒张晚期血流速度A峰(A)、E/A、E峰减速时间(DT)]均明显改善(P<0.01),治疗组与对照组比较差异有统计学意义(P<0.05)。不同心功能组6MWT距离均明显增加,差异有显著统计学意义(P<0.01),治疗组与对照组比较差异有统计学意义(P<0.05)。结论高压氧治疗,能显著的改善HFNEF患者的心功能状态,改善心衰患者的运动耐量。6 MWT作为量化评价指标,客观、实用,是一种值得推广的评价心力衰竭治疗效果的方法。
Objective To explore 6 - minutes walk test(6MWT) evaluation of hyperbaric oxygen on heart failure with normal ejection fraction (HFNEF) in heart failure patients with normal cardiac function. Methods A total of 100 heart failure patients with normal ejection frac- tion were randomly divided into the control group (n = 50 ) and the HBO group (n = 50). The control group were given the routine therapy, The treatment group on the basis of conventional drug treatment received hyperbaric oxygen treatment. The color doppler ultrasound before and after treatment for 3 months were used to measure left ventricular diastolic function. According to different heart function classification line 6 MWT. The correlation of the two indexes were investigated. Results After treatment of the two groups, left ventricular diastolic function improved significant- ly (IVRT, E, A, E/A, DT) ( P 〈0.01). Compared with the control group, the difference was statistically significant ( P 〈0.05). 6 MWT distance in different heart function group were significantly increased. There are significant difference statistically significant( P 〈 0.01 ). Com- pared with the control group, the difference was statistically significant ( P 〈 0.05 ). Conclusion Hyperbaric oxygen therapy can significantly improve left ventricular ejection fraction of normal cardiac function in patients with heart failure. Exercise tolerance in heart failure patients can be improved.
