2009～2011年海南省农村妇女乳腺癌检查项目实施效果评价

Evaluation of Breast Cancer Screening Project for Hainan's Rural Women from 2009 to 2011

目的:在社会调查和数据收集的基础上,对海南省农村妇女乳腺癌检查项目实施3年的效果进行评价,为项目可持续开展提供科学依据。方法:分析乳腺癌检查数据库、督导、培训记录和财务等资料,估算项目实施总成本,计算项目疾病检出情况和质量调整寿命年(QALY),对项目成本效用比做出评估。结果:共检出良性肿瘤患者108例,检出乳腺癌9例,乳腺癌检出率22.54/10万;挽救17.1个寿命年,总体成本效用为14万元/寿命年。结论:项目完成情况较好,但效用有待提高;项目提高了基层妇幼人员技术和农村妇女防癌知识水平;项目管理存在一定缺陷,影响项目质量。

Objective Based on social surveys and relevant statistics,the performance and progress of the rural women breast cancer screening project in Hainan Province from 2009 to 2011 were evaluated, so as to provide scientific basis for its sustainable development. Methods For carrying out the evaluation, four steps were conducted,including： （ 1 ） analyzing the data resources as- sociated with breast cancer screening database, supervision, training records and financial information; （ 2 ） estimating the total cost on the project; （3） computing detection of disease and quality-adjusted life year （QALY） and （4） assessing cost-utility rati- o of the project. Resuhs：108 cases with benign tumors,9 cases with breast cancer were detected （the detection rate is 22.54 per 100000） ;saved 17.1 life years, the cost-utility ratio is 140000 yuan per life year. Conclusions Evaluation results show that the project was generally completed well, nevertheless the utility needs to be perfected further. Meanwhile, the basic health staff bene- fited from the project in view of their skills, as well as the rural women who improved their knowledge level of cancer prevention. However,it is worth to mention that some defects in management affected the quality of the project.
