二甲双胍对奥氮平致体质量增加的精神分裂症患者血清同型半胱氨酸水平的影响 被引量:11

摘要 目的观察二甲双胍干预奥氮平致精神分裂症患者体质量增加过程中血清同型半胱氨酸的变化。方法将奥氮平治疗后出现体质量显著增加的精神分裂症患者随机分组,在维持奥氮平治疗的同时,二甲双胍组(n=24)加用二甲双胍(750mg/d)药物干预,对照组(n=25)不予干预。分别于治疗前、治疗后4、8周,测定体质量、体质量指数(BMI)、血清同型半胱氨酸(Hcy)水平。结果①治疗4、8周时二甲双胍组的体质量(分别t=2.175,2.696;P均<0.05);BMI(t=2.232,2.733;P均<0.05)均较治疗前显著下降,对照组则无显著变化;②治疗4,8周时二甲双胍组的血清Hcy水平均较治疗前显著下降(t=2.356,2.812;P均<0.05);对照组则无显著变化;③2组治疗4、8周时体质量、BMI的变化值与同期血清Hcy水平的变化值无显著的相关性。结论二甲双胍在降低奥氮平致体质量增加的同时,也降低患者血清Hcy水平,但作用机制可能不同。 Objective To assess the efficacy of metformin on serum homocysteine level in schizophrenia patients with olanzapine-indueed weight gain. Methods Schizophrenia patients with olanzapine-induced weight gain were randomly divided into two groups receiving continuative olanzapine treatment, one with mefformin intervention ( 750mg/d, n = 24), the other with none intervention ( n = 25 ). The body weight,body mass index(BMI) and serum homocysteine level were measured before and week 4,8 after treatment. Results Body weight( t = 2. 175,2. 696 ;P 〈 0.05 ), BMI( t = 2. 232,2. 733 ;P 〈 0.05 ) and serum homocysteine level( t = 2. 356,2. 812 ;P 〈 0.05 ) were significantly decreased in mefformin group and not significantly changed in subjects without mefformin intervention ( P 〉 0.05 ). The diversify of body weight or BMI before and week 4,8 after treatment were not significantly correlated with the alteration of serum homoeys- teine level(P 〉 0. 05). Conclusion Metformin addition therapy induces a decrease not only in body weight and BMI, but also in serum homocysteine level in schizophrenia patients with olanzapine-induced weight gain, which possibly has differential action.
