Effects of Different Drug Intervention on the Duration of Vaginal Bleeding after Painless Induced Abortion

目的探讨单纯应用异丙酚与异丙酚+阿托品和单纯应用笑气与笑气联合米索前列醇对无痛人工流产术后阴道出血时间的影响。方法随机选择400例早孕自愿要求进行无痛人工流产术者,将400例孕妇随机分为4组每组100人,异丙酚+阿托品组在静推异丙酚之前静推阿托品;异丙酚组直接静脉推注异丙酚注射液;笑气联合米索前列醇组在术前2小时空腹口服米索前列醇,术中给予笑气吸入;笑气组仅在术中给与笑气吸入。结果异丙酚+阿托品组和异丙酚组比较(χ2=0.4134,P<0.05)有显著差异,笑气联合米索前列醇组和笑气组比较有显著差异(χ2=14.22,P<0.05),异丙酚+阿托品组和笑气联合米索前列醇组比较两组有显著差异(χ2=4.7138,P<0.05)。结论无痛人工流产术前加用阿托品或米索前列醇,这两种方法弥补了单纯静脉麻醉和吸入麻醉的不足,缩短了术后阴道出血时间,在这4种方法中笑气联合米索前列醇在缩短无痛人工流产术后阴道出血时间效果最佳。

Objectives To study the different effects between application of propofol only and of propofol ＋ atropine,and that of nitrous oxide only and nitrous oxide combined with misoprostol for vaginal bleeding duration after painless induced abortion. Methods A total of 400 cases of early pregnant women,voluntarily for painless induced abortion,were randomly sdected and divided into four groups and each group of 100 cases. Propofol ＋ atropine group were given intravenous atropine before intravenous propofol;Propofol group were only given intravenous injection of propofol ; Nitrous oxide ＋ misoprostol group took oral misoprostol with empty stomach 2 hours before op- eration as well as were given nitrous oxide inhalation during operation. Nitrous oxide group were only given nitrous oxide inhalation during operation. Results The difference between propofol ＋ atropine group and propofol group was significant （ X2 = 0. 4134, P 〈 0.05 ）. The difference between nitrous oxide ＋ misoprostol group and nitrous oxide group was significant（ X2 = 14.22, P 〈 0.05 ）. And the difference between propofol ＋ atropine group and nitrous oxide ＋ misoprostol group was significant （X2 = 4. 7138, P 〈 O. 05 ）. Conclusion The two methods of plus atropine or misoprostol before painless induced abortion made up for the lack of single intravenous anesthesia or inhala- tion anesthesia as well as shorten the duration of vaginal bteeding after operation. Among the four methods, nitrous oxide ＋ misoprostol has the best effect on the duration of vaginal bleeding after painless induced abortion.
