微创埋线用于针药复合麻醉对肺切除手术麻醉效果的影响

Influence of Microinvasive Catgut Embedding on the Effect of Combined Acupuncture and Medicine Anesthesia During Pneumonectomy

目的比较在针药复合麻醉肺切除手术中运用微创埋线与全麻的麻醉效果,为针麻肺切除手术探索新的方法提供临床依据。方法将50例肺切除手术患者随机分为治疗组和对照组,每组25例。治疗组术前予以微创埋线,取穴华佗夹脊(T4、T6、T8)与背俞穴(肺俞、心俞、膈俞)、曲池、足三里。对照组采用全麻。记录两组术中11个时间点的生理指标,计量术中麻醉药物实际使用量等指标。结果两组术中11个时间点心率、收缩压与舒张压比较,差异均无统计学意义(P>0.05)。治疗组术中芬太尼追加量与对照组比较,差异具有统计学意义(P<0.05)。两组术中丙泊酚及罗库溴铵追加量比较,差异均无统计学意义(P>0.05)。结论微创埋线相对于全麻方案,能减少手术中镇痛药物的使用剂量,微创埋线与药物在肺切除手术中具有良好的协同作用。

Objective To compare the effects of microinvasive catgut embedding plus combined acupuncture and medicine anesthesia versus general anesthesia during pneumonectomy to provide a clinical basis for exploration of a new method for pneumonectomy under acupuncture anesthesia. Methods Fiftypneumonectomy patients were randomly allocated to treatment and control groups, 25 cases each. Microinvasive catgut embedding was performed in the treatment group before the operation. Huatuojiaji points （T4, 6, 8） and Back-Shu points （Feishu, Xinshu and Geshu） were selected. General anesthesia was performed in the control group. Physiological indices at 11 time points and the given dosages of anesthetics were recorded during the operation, Results There were no statistically significant differences in heart rate, systolic pressure and diastolic pressure at 11 time points during the operation between the two groups （P〉0.05）. There was a statistically significant difference in the intraoperatively added dosage of fentanyl between the treatment and control groups （P〈0.05）. There were no statistically significant differences in the intraoperatively added dosages of propofol and vecuronium between the two groups （P〉0.05）. Conclusions in comparison with general anesthesia, microinvasive catgut embedding can reduce the dosages of analgesics given intraoperatively Microinvasive catgut embedding and medicine have a good synergistic effect during pneumonectomy.
