摘要
目的探究丙型肝炎病毒(HCV)不同感染阶段,外周血单核细胞(PBMC)干扰素α/β受体2(IFNAR2)mRNA表达水平,并研究其与干扰素(IFN)疗效的关系。方法 58例丙肝抗体阳性患者,根据临床表现分为3组:急性肝炎组(4例),慢性肝炎组(46例),肝硬化组(8例)。15名健康志愿者作为对照组。应用密度梯度离心法将外周血单核细胞分离,应用RT PCR扩增IFNAR2 mRNA。结果 58例丙型肝炎患PBMC IFNAR2 mRNA检出率为87.9%,而对照组为33.3%,差异具有统计学意义(P<0.05);慢性丙型肝炎组较肝硬化组检出率高(93.5%和50%,P<0.05),2组间PBMC HCV RNA的阳性率无差别(P>0.05)。PBMC IFNAR2 mRNA水平与干扰素疗效相关(P<0.05)。结论丙型肝炎病毒感染可上调PBMC IFNAR2 mRNA的表达,慢性丙型肝炎患者PBMC IFNAR2 mRNA表达高于肝硬化患者,其表达可能与病毒载量无关,而与干扰素疗效相关。
Objective To investigate the differences in interferon-α/β receptor 2 (IFNAR2) mRNA expression level in peripheral blood mononu- clear cells ( PBMC ) at different stages of hepatitis C virus ( HCV ) infection, and determine whether it has any correlation with the eft;ectiveness of in- tcrteron therapy. Methods A total of 58 patients, positive for anti-HCV antibodies, were recruited for the study and divided into three groups de- pending on their clinical symploms : acute hepatitis ( 4 cases ), chronic hepatitis (46 cases ) and liver cirrhosis ( 8 cases ). 15 volunteers served as healthy controls. PBMC were purified hy density gradient centrifugation and IFNAR2 mRNA was amplified by a reverse transcription- polymerase chain reactiun ( RT-PCR ) assay. Results The detection rate of IFNAR2 mRNA was 87.9% ( 51/58 ) in the PBMC of HCV patients, which was significantly higher than the eontrul group ( 33.3%, P 〈 0.05 ). While the positive rate was 93.5% ( 43/46 ) in the chronic hepatitis group, which was sig- nificanny higher than the liver cirrhosis group ( 50%, P 〈 0.05 ). Fm'theimole, there is no significant ditlb.rence in the positive rates for HCV RNA d PBMC among three groups ( P 〉 0.05 ). There was no correlation between the positive rate ofIFNAR2 mRNA in PBMC and the vital load dHCVR- NA in serum ( P 〉 0.05 ). However, higher expression of IFNAPt2 mRNA in the PBMC did correlate with the effectiveness of interferon therapy ( P 〈 0.05 ). Conclusion HCV intioion up-regulates the expression of IFNAR2 mRNA in PBMC. IFNAR2 mRNA expression in the chronic hepatitis group was higher than that in the liver cirrhosis group, which is significantly correlated with the effectiveness of interferon therapy, but independent of the viral load.
