妊高症患者胎儿静脉导管分流率超声检测及其临床意义 被引量:4

摘要 目的:研究中晚孕期胎儿静脉导管分流率的正常参考值范围及研究妊高症患者胎儿脐静脉导管分流率及相关临床意义。方法:选择2009年12月至2012年5月入院的220例接受超多普勒超声诊断的14~41孕周胎儿的静脉导管及脐静脉各项血流动力学参数,并建立孕周与各项检测指标的直线相关方程。其中220例胎儿中180例正常胎儿,20例母体患轻中度妊高症胎儿、20例母体患重度妊高症胎儿,各项血流动力学参数值进行标准正态分布转换后做进一步比较。结果:(1)静脉导管内径、静脉导管血流量、脐静脉血流量随孕周的增加而增加,静脉导管分流率随孕周的增加而减低;(2)多元逐步回归分析结果显示脐静脉血流量对分流率的贡献最大;(3)随孕妇妊高症炎症程度增加,胎儿脐静脉血流量逐渐降低,静脉导管血流量及静脉导管分流率逐渐增加,其中以静脉导管分流率增加最为显著。结论:利用彩色超声多普勒检测胎儿静脉导管分流率是检测孕妇妊高症严重程度的重要参考指标。 Objective:To study the late pregnancy fetal venous catheter FenLiuLv normal reference range and research RenGaoZheng patients fetal umbilical vein catheter FenLiuLv and related clinical significance. Methods:Choose from December 2009 to May 2012 to 220 patients with super doppler uhrasound diagnosis of 14 - 41 gestational weeks fetal venous catheter and umbilical vein the hemodynamic parameters, and establish gestational weeks and the detection index of the linear correlation equation. 220 cases of fetus in 180 cases of normal fetus, twenty patients with mild -to -moderate RenGaoZheng maternal fetal, twenty patients with severe RenGaoZheng maternal fetal, the hemodynamic parameters value standard normal distribution after converting to do further comparison. Results:Venous catheter inside diameter, venous catheter blood flow, umbilical venous blood flow with gestational weeks increases, FenLiuLv venous catheter with gestational weeks of decreases; Multivariate stepwise regression analysis results show that umbilical venous blood flow to FenLiuLv contribute; Pregnant women with RenGaoZheng inflammation degree increase, fetal umbilical venous blood flow to gradually reduce, intravenous catheter blood flow and venous catheter FenLiuLv gradually increase, of which the most significant increase FenLiuLv venous catheter. Conclusions : Using color doppler ultrasound in detecting fetal venous cath- eter FenLiuLv is testing the severity of pregnant women RenGaoZheng important reference index.
