摘要 目的:探讨CT、MRI对巨大(>5.0 cm)原发性、无功能性肾上腺肿瘤的诊断及鉴别诊断价值。方法:对49例经临床及病理证实的巨大原发性无功能性肾上腺肿瘤的CT、MRI表现进行回顾性分析;对4例无功能性腺瘤患者、8例非腺瘤患者作化学位移成像(CSI)检查。结果:CT、MRI对肾上腺肿瘤定位、定性及判断肿瘤组织学类型的准确率分别为91%、93%、77%和94%、94%、79%。3例腺瘤患者在MRI反相位(OP)图像上的信号强度(SI)明显下降,SI指数平均为55%;8例非腺瘤和1例腺瘤患者的信号强度无明显下降。结论:CT、MRI对巨大原发性无功能性肾上腺肿瘤的定位和定性准确率较高,但其判断肿瘤组织学类型有一定的限度。 Objective:To investigate the value of CT and MRI in the diagnosis and differential diagnosis of the large primary nonfunctional adrenal tumors(5.0 cm).Methods:CT and MRI features of the large primary nonfunctional adrenal tumors verified with clinical and pathology were investigated retrospectively in 49 cases.MR chemical shift imaging(CSI) was done in 12cases(nonfunctional adenoma 4 cases and non-adenoma 8 cases).Results:The accuracies of location,qualitatively diagnoses(differentiating benign tumor from malignant tumor) and tissue characterization of CT and MRI were 91%,93%,77% and 94%,94%,79% respectively.Three nonfunctional adenomas exhibited the decrease in signal intensity(SI) on MR opposed-phase(OP) images,the average of SI index was 55%,non-adenomas and 1 nonfunctional adenoma showed no loss of signal intensity on OP images.Conclusion:CT and MRI are useful in the location and qualitatively diagnoses of the large primary nonfunctional adrenal tumors,but they have some limitation in the tissue characterization of tumors.
