补虚泄实针刺法结合卒中单元模式对脑卒中后肩手综合征患者上肢功能的影响

Effect of Tonifying Deficiency and Reducing Excess Needling Combined with Scheme for Individual Stroke Units on Functions of Upper Limbs with Patients of Shoulder-Hand Syndrome after Apoplexy

目的 :观察补虚泄实针刺法结合卒中单元模式对脑卒中后肩手综合征患者上肢功能的影响。方法 :将80例患者随机分为治疗组和对照组,每组各40例。治疗组采用补虚泄实针刺法结合卒中单元模式进行治疗,对照组采用普通针刺法结合卒中单元模式进行治疗,4周后观察两组患者上肢Fugl-Meyer运动功能评分及总体疗效。结果 :治疗组总有效率为92.5%,对照组的总有效率为87.5%(P<0.05);两组治疗后上肢Fugl-Meyer运动功能评分较治疗前显著增加(均P<0.01),治疗组明显优于对照组(P<0.01)。结论 :补虚泄实针刺法结合卒中单元模式和普通针刺法结合卒中单元模式均能能明显改善脑卒中后肩手综合征患者的上肢运动功能,对脑卒中后肩手综合征有明显的治疗效果,但补虚泄实针刺法结合卒中单元模式的疗效更优一些。

Objective ： To observe the influence of tonifying deficiency and reducing excess needling combined with the scheme for individual stroke units on functions of upper limbs with patients of shoulder- hand syndrome after apoplexy. Methods： Eighty patients were randomly divided into a treatment group （ 40 cases ） and a control group （ 40 cases ） .Tonifying deficiency and reducing excess needling combined with the scheme for individual stroke units was applied in the treatment group, while ordinary needling combined with the scheme for individual stroke units to the control group.Fugl-Meyer＇s scoring reflecting moving functions of upper limbs and therapeutic effects were made after 4 weeks of treatment in both groups. Results ： The total effective rate was 92.5% and 87.5% in the treatment group and control group respectively, showing superiority of the former to the latter （ P〈0.05 ） .After treatment, moving functions of upper limbs shown by Fugl-Meyer＇s scoring were improved in both groups （ P〈0.01 ）, but the treatment group was better than the control group （ P〈0.01 ） .Conclusion ： The tonifying deficiency and reducing excess needling combined with the scheme for individual stroke units and ordinary needling combined with the scheme for individual stroke units are effective in improving the moving functions of upper limbs.Both have obvious therapeutic effect on shoulder-hand syndrome after apoplexy, yet the former is better than the latter.
