脑血管畸形的治疗进展

Progress in Treatment of Vascular Malformations

脑血管畸形的治疗方法有显微手术治疗、血管内栓塞治疗、立体定向放射治疗。由于神经影像技术突飞猛进的发展,各种治疗方法都有了明显的提高。目前临床医师对于具体的病例选择何种治疗仍存在一定争议。该文通过对脑血管畸形(主要是动静脉畸形)的血管构筑、血流动力学特点、出血风险、自然病史、影像学表现等进行详细阐述,分析上述三种治疗方法各自的优缺点,认为上述2种或3种治疗方法的联合治疗,互相补充,扬长避短,可以提高患者治愈率,降低致残率和病死率。

The treatment methods of cerebral vascular malformations are microscopic operation treat- ment, endovascular therapy, and stereotactic radiotherapy. Due to the spurt development of neuroimaging tech- nology, various treatment methods have been significantly improved. At present, the choice of treatment for the specific case is still controversial. Here is to analyze the pros and cons of the above mentioned three methods through detailed elaboration on the arteriovenous malformation vessels construction, hemodynamic characteris- tics, the risk of bleeding, natural illness history, imaging findings etc. of the cerebrovaseular malformation, thinking that the combination of the two or three treatment methods can complement each other, foster strengths and circumvent weaknesses,which can improve the cure rate and reduce morbidity and mortality.
