糖皮质激素联合鼻内镜术治疗鼻息肉的疗效观察

Curative Effect Observation of Glucocorticoid Combined with Nasal Endoscopic Surgery for Nasal Polyp

目的观察糖皮质激素联合鼻内镜术治疗鼻息肉的临床疗效。方法选取2008年2月至2011年2月在潜江市中心医院诊治的134例鼻息肉患者按数字法随机分为两组各67例,进行回顾性分析,对照组仅采取鼻内镜下息肉切除术治疗,观察组在对照组治疗基础上加用糖皮质激素治疗。比较两组患者治疗总有效率和手术时间、术中出血量、住院时间等情况变化。结果观察组治疗总有效率为91.0%,显著高于对照组的77.6%,两组的疗效比较差异有统计学意义(Z=2.334,P<0.05);观察组患者手术时间、术中出血量和住院时间较对照组均显著减少(P<0.05);观察组复发率(6.0%)显著低于对照组(22.4%)(χ2=8.723,P<0.05)。结论糖皮质激素联合鼻内镜术治疗鼻息肉的疗效显著、复发率低,值得临床推广应用。

Objective To observe the curative effect of glucocorticoid combined with nasal endoscopic surgery in treatment of nasal polyps. Methods From Central Hospital of Qianjiang during Feb： 2008 and Feb. 2011,134 patients with nasal polyps were randomly divided into two groups, the control group （n = 67）was only treated with nasa] endoscopic polypectomy, and the observation group （n = 67 ）was treated with glucocorticoid on the basis of the control group treatment. The total efficiency and operation time, intraoperative blood loss, hospi- tal stay and other changes of patients in two groups were compared. Results The total efficiency of observation group was 91.0%, which was significantly higher than that of control group （77.6%）, there was statistically significant compare of the curative effect（ Z = 2. 334, P 〈 0.05 ）. The operation time, intraoperative blood loss and duration of hospitalization of the observation group were significantly decreased than that of the control group （ P 〈 0.05 ）. The recurrence rate of the observation group（6.0% ） was significantly lower than that of the control group（22.4% ） （ X2 = 8. 723, P 〈 0.05 ）. Conclusion The treatment of glucocorticoid combined with nasal endoscopic surgery for nasal polyps has an obvious curative effect, with low recurrence rate, which is worthy of clinical application.
