腰椎间盘突出症患者外周血T淋巴细胞亚群水平变化及其意义

Correlation of T lymphocyte subsets in the peripheral blood and clinical symptoms of patients with lumbar disc herniation

目的观察腰椎间盘突出症(LDH)患者较之健康人外周血T淋巴细胞亚群水平的变化,并探究与其临床症状的相关性。方法选取伴有下肢神经根性疼痛的LDH患者21例作为研究组,同期选取健康人员21例作为对照组,利用流式细胞仪测定两组人员外周血T淋巴细胞亚群水平,并对研究组患者进行直腿抬高试验、VAS疼痛评分以及简体中文版Oswestry功能障碍指数问卷评定,利用SPSS软件进行相关结果的分析对比。结果研究组患者CD4+T淋巴细胞水平较健康对照组升高,差异有统计学意义(P<0.05);研究组患者CD4+/CD8+比值较健康对照组大,差异有统计学意义(P<0.05);而两组CD3+及CD8+T淋巴细胞水平差异无统计学意义(P>0.05)。LDH患者CD3+T淋巴细胞亚群百分比与SLR角度的相关系数r等于-0.554;CD4+T淋巴细胞亚群百分比与SLR角度相关系数r等于-0.556,两组数据均在0.01水平上呈负相关。未发现VAS及SCODI评分与T淋巴细胞亚群水平之间存在有统计学意义的相关性。结论 LDH患者外周血CD4+T淋巴细胞亚群水平较正常健康人升高;T淋巴细胞亚群水平与LDH患者临床症状之间存在关联性;T淋巴细胞在腰椎间盘突出的自身免疫机制中起重要作用。

Objective To observe changes in peripheral T lymphocytes of patients with lumbar disc herniation,and look for any clinically relevant links between T lymphocyte subsets in the peripheral blood and clinical symptoms of lumbar disc herniationos.Methods Blood samples were collected from 21 patients with lumbar disc herniationos（study group）,these patients come from Rehabilitation Department of the Second Hospital of AnHui Medical University,and blood samples were also collected from 21 healthy controls.T lymphocytes subsets in the peripheral blood of the study group and control group were detected by cytometer.Visual Analogue Scale（VAS） 、straight leg raising test（SLR） and Simplified Chinese Version of Oswestry Disability Index（SCODI） of study group were assessed.Statistical analysis were performed by SPSS.Results Percentages of CD4＋ T lymphocytes and ratio of CD4＋/CD8＋ in the study group were signi？ cantly higher than in the control group（P＆lt;0.05）.No statistically signi？cant differences of the percentages of CD3＋ and CD8＋T lymphocytes was observed between study group and control group.Negative correlation was observed between the angle of SLR and the percentages of CD3＋ 、CD4＋ T lymphocytes（r=-0.554 and-0.556,P＆lt;0.01）.Conclusion Our results suggest T lymphocytes may play an important role in the autoimmune response of lumbar disc herniationos,whichis an influencing factor participate in developing symptoms in patients with lumbar intervertebral disc herniation.
