后房型有晶体眼人工晶体植入术与Lasik术矫正高度近视的疗效比较 被引量:14

摘要 目的比较后房型有晶体眼人工晶体植入术和Lasik术矫正高度近视的效果及术后视觉质量。方法收集在我科行Lasik术和后房型有晶体眼人工晶体植入手术治疗一年的高度近视患者。分为后房型有晶体眼人工晶体植入术组(ICL组,17例,34眼)和Lasik组(21例,35眼),随访1年,观察两组病例术后1年的裸眼视力、最佳矫正视力、眼压、对比敏感度及并发症,比较两组的有效性和安全性。结果 Lasik组有效性指数平均值为(93.32±15.69),ICL组为(104.78±18.73),ICL组明显优于Lasik组。Lasik组安全性指数平均值为(100.61±11.62),ICL组为(108.89±17.75),ICL组明显优于Lasik组。Lasik组术后明暗环境下对比敏感度在各个频段均较ICL组低,在白天空间频率段1.5 cpd、3 cpd、6 cpd、12 cpd和夜晚1.5 cpd、3 cpd,差异有统计学意义(P<0.05)。结论有晶体眼后房型人工晶体植入矫正高度近视术后的安全性指数,有效性指数及对比敏感度优于Lasik组,是一种安全有效的高度近视矫正方法,术后视觉质量优于Lasik。 Objective To compare the efficacy and visual quality between Lasik group and phakic posterior chamber intraocular lens implantation group with high degree myopia. Methods The patients were divided into two groups: Lasik group (21 patients, 35 eyes) and phakic posterior chamber intraocular lens implantation group (17 patients, 34 eyes). The efficacy, safety and contrast sensitivity were com- pared between two groups. Results The mean efficacy index was (93.32 ±15.69 ) in Lasik group and ( 104.78 ± 18.73 ) in ICL group. The mean safety index was (100.61 ± 11.62 )in Lasik group and (108.89 ± 17.75 )in ICL group. Contrast sensitivity of every space frequency in both photopic and mesopic in Lasik group were lower than those in ICL group, and there were statistically significant differences at 1.5, 3, 6, 12 cpd in photopic and 1.5, 3 cpd in mesopic. Conclusion The efficacy, safety and contrast sensitivity of patients treated by phakic pos terior chamber intraocular lens implantation were superior to Lasik group. Phakic posterior chamber intraocular lens implantation is a safe and efficient correction for high degree myopia. Visual quality is better with phakic intraocular lens implantation postoperatively.
