间质流膏联合泼尼松治疗气阴两虚、瘀毒阻络型特发性间质性肺炎随机平行对照研究

The Random Parallel Control Study of the Interstitial Flow Cream Combined with Prednisone Therapy for Qi Yin Deficiency Type and Blood Stasis Poison Resistance Type Winding Idiopathic Interstitial Pneumonia

[目的]观察间质流膏联合泼尼松治疗气阴两虚、瘀毒阻络型特发性间质性肺炎疗效。[方法]使用随机平行对照方法,将90例门诊患者按随机数字表方法随机分为三组。间质流膏组(A组)30例间质流膏(党参24g,黄芪18g,白术、茯苓各12g,沙参20g,水蛭研末冲服6g,蜈蚣研末冲服2条,丹参21g,桃仁、川芎、地龙各15g,黄芩12g,白花蛇舌草、薏苡仁各30g,蒲公英24g,浙贝15g,莪术12g,山慈菇21g,甘草12g,连翘15g,鱼腥草18g。兼痰浊:寒痰:加白前粉冲服12g;热痰:加浙贝粉冲服15g。兼毒邪:加羚羊角粉冲服3g。兼肾虚:加胡桃粉冲服15g。兼咯血:加云南白药冲服、三七粉冲服各3g。),30mL/次,3次/d;间质流膏激素联合组(B组)30例醋酸泼尼松片,0.5mg/kg·d,连续治疗4周后改为0.25mg/kg·d,间质流膏治疗同A组;激素组(C组)30例,醋酸泼尼松片治疗同B组。连续治疗12周为1疗程。观测临床症状、CRP评分、不良反应。连续治疗1疗程,判定疗效。[结果]临床评分(C)各组均明显降低(P<0.01),A、B两组均优于C组(P<0.01),A、B两组间无明显差异(P>0.05);影像学评分(R)各组间无明显差异(P>0.05);生理评分(P)各组均明显降低(P<0.01),B组均优于A、C两组(P<0.01);总评分(CRP)各组均明显降低(P<0.01),B组均优于A、C两组(P<0.01)。[结论]间质流膏联合泼尼松治疗气阴两虚、瘀毒阻络型特发性间质性肺炎疗效满意。

[Objective]To observe the effect of interstitial flow cream combined with prednisone treatment for qi Yin deficiency type and blood stasis poison resistance type of idiopathic interstitial pneumonia curative.[Methods]Using random parallel control method,divided 90 clinic patients randomly into three groups according to random number table method,Interstitial flow cream group（group A）30 cases of interstitial flow cream（Codonopsis pilosula 24g,Astragalus membranaceus 18g,Atractylodes macrocephala koidz、poria cocos each 12g,straight ladybell 20g,Pulverization Oral in Pulverization 6g,2 pieces of centipede Oral in Pulverization,Salvia miltiorrhiza 21g,Peach kernel,rhizoma ligustici wallichii,lumbricus each 15g,Radix scutellariae 12g,Spreading hedyotis herb,semen coicis each 30g,Dandelion 24g,Thunberg Fritillary Bulb15g,rhizoma zedoariae 12g,mountain arrowhead 21g,Licorice is 12 g,forsythia,15 g,cordata 18 g。With phlegm turbidity symptom：cold phlegm：add radix cynanchi powder Oral 12g;Hot phlegm：add Thunberg Fritillary Bulb powderOral 15g。With toxin factor：add antelope horn powder Oral 3 g。With kidney：add walnut powder Oral 15g。With hemoptysis：add yunnan baiyao Oral、notoginseng powder Oral each 3g。）,30ml/time,3 times/d,Interstitial flow cream combined hormone group（group B）30 cases of prednisone acetate tablets,0.5 mg/kg·d,continuous treatment of 4 weeks reduced to 0.25 mg/kg·d,Interstitial flow cream treatment is same as group A,Hormone group（group C）30 cases with prednisone acetate tablets treatment is same as group B,continuous treatment of 12 weeks as 1 course of treatment. Observing the clinical symptoms,CRP score and adverse reactions,12 weeks continuous treatment is as 1 treatment course,then to judge the curative effects.[Results]The clinical score（C）groups were significantly decreased（P0.01）,A,B two group was better than the C group（P0.01）,A,B no significant difference between the two groups（P0.05）;imaging score（R）had no significant difference among the three groups（P0.05）;physiological score（P）groups were significantly decreased（P0.01）,group B were better than A,C two groups（P0.01）;total score（CRP）groups were significantly decreased（P0.01）,group B were better than A,C two groups（P0.01）.[Conclusion]The effect of Interstitial flow cream combined prednisone treatment of qi Yin deficiency type and blood stasis poison resistance type winding idiopathic interstitial pneumonia curative is satisfied.
