益气活血中药对三维适形放疗联合吉西他滨化疗治疗中晚期胰腺癌疗效影响的研究

Effect of Qi Replenishing and Blood Circulation Activating Drugs in Treatment of Middle Advanced Pancreatic Cancer by Three-dimensional Conformal Radiotherapy and Chemotherapy

目的探讨益气活血中药对三维适形放疗联合吉西他滨化疗治疗中晚期胰腺癌疗效的影响。方法 106例中晚期胰腺癌患者随机分成两组,第1组48例,行三维适形放疗联合吉西他滨化疗;第2组58例,在第1组放化疗基础上,给予益气活血中药。观察两组上消化道反应、腹泻、腹痛、骨髓抑制及生活质量改善情况,同时通过疗效评价法观察近期有效率,分析患者的1、2年生存率。结果①第1组上消化道反应、腹泻、腹痛、骨髓抑制及生活质量改善情况分别为45.83%、18.75%、56.76%、58.33%、35.42%,第2组分别为24.14%、5.17%、24.14%、84.44%、53.45%,两组差异均有统计学意义(P<0.05)。②第1组、第2组近期有效率分别为56.3%(27/48)、67.2%(39/58),两组差异有统计学意义(P<0.05)。③1、2年生存率第1组分别为36.8%、16.6%,第2组分别为75.3%、28.2%,第2组总体生存率优于第1组(χ2=9.205,P=0.002)。结论益气活血中药能减轻放化疗所致的消化道反应,提高临床症状有效率,延长1、2年生存期。

Objective To investigate the effect of Qi replenishing and blood circulation activating drugs （QRBCAD） in treatment of middle advanced pancreatic cancer by three-dimensional conformal radiotherapy and chemotherapy. Methods A total of 106 patients with locally advanced pancreatic cancer were randomly divided into two groups. Group 1 （48 patients） was treated by radiotherapy and chemotherapy. Group 2 （58 patients） was treated by the same therapy and additional QRBCAD medication. The Upper gastrointestinal tract reaction, diarrhoea, abdominal pain, bone marrow suppression situations, the changes of quality of life, and the one- and two-year survival rates of two groups were ob- served and compared. Results （1） The improve rate of upper gastrointestinal tract reaction, diarrhoea, abdominal pain, bone marrow suppression, and the quality of life in group 1 was 45. 83%, 18. 75% ,56. 76, 58.33% and 35.42% respec tively, and in group 2 was 24. 14%, 5.17%,24. 14%, 84. 44 and 53. 45% respectively （significantly different between two groups, P〈0. 05）. The short term effective rate in group l and 2 was 56. 3% and 67.2% respectively （P〈 0. 05）. The 1 and 2 year survival rate in group 1 was 36. 8% and 16. 6%, and in group 2 75. 3% and 28. 2% respectively （significantly different between two groups, P〈0. 05）. Conclusion QRBCAD can alleviate the gastroenteric reaction caused by radiotherapy and chemotherapy, raise the clinical effective rate, and prolong the 1 and 2 year survival rate of patients.
