癌胚抗原mRNA、人中性粒细胞多肽1-3联合检测在结直肠癌诊断中的价值

Preliminary study of the diagnosis of colorectal cancer by single and combined detection with peripheral blood CEAmRNA and HNP1-3

摘要 目的:探讨外周血癌胚抗原信使核糖核酸(CEAmRNA)、中性粒细胞多肽1-3(HNP1-3)单一检测及联合检测在结直肠癌诊断中价值。方法:选取结直肠癌70例患者为观察组,另选良性疾病患者70例为对照组。分别采用Nested RT-PCR法检测CEAmRNA,ELISA法检测HNP1-3,观察两组患者CEAmRNA及HNP1-3的表达。结果:CEAmRNA对结直肠癌的诊断敏感度为52.86%,特异度为98.57%;HNP1-3对结直肠癌的诊断敏感度为60%,特异度为98.57%。两者联合检测时至少1项阳性敏感度为77.14%,特异度为97.14%,2项均阳性敏感度为42.85%,特异度为100%。CEAmRNA、HNP1-3的表达与淋巴结转移及Dukes分期相关。结论:在结直肠癌诊断中,外周血CEAmRNA和HNP1-3二者平行相关检测具有更高价值和诊断意义。 Objective: To explore the diagnosis of colorectal cancer by the single detection,parallel and series detection with peripheral blood CEAmRNA and HNP1-3. Methods: The expression of CEAmRNA was detected by using Nested RT-PCR in 70 patients with colorectal cancer (the experimental group) and in 70 normal subjects (the control group). The expression of HNP1-3 was detected by using ELISA in both groups. Results:The sensitivity of CEAmRNA and HNP1-3 in the experimental group was 52.86% and 60%, respectively. The specificity of CEAmRNA and HNP1-3 in the experimental group was 98.57%. A higher sensitivity (77.14%)was shown when either of the CEAmRNA and HNP1-3 was positive. A higher specificity(100%)was found when both of the CEAmRNA and HNP1-3 were positive. The expressions of CEAmRNA and HNP1-3 were not significantly correlated with the patient’s sex, age, position of tumor or differentiation level of cancer cells, but with the clinical stage and lymph node metastasis. Conclusion:In the diagnosis of colorectal cancer, the single detection plays a certain role. But the parallel and series detections are more valuable and diagnostically significant.
