2型糖尿病肾病的代谢特点及其相关研究 被引量:3

Relative research on metabolism features of Type II diabetic nephropathy

摘要 目的观察普通2型糖尿病和糖尿病肾病患者临床各项生理生化指标变化,深入了解各项指标在糖尿病肾病患者诊疗中的意义。方法通过对72例普通2型糖尿病患者和40例2型糖尿病肾病患者临床资料回顾收集整理,记录患者身高、体重等各项生理指标并运用生化蛋白分析仪检测患者糖化血红蛋白、肾功能、血脂、尿微量白蛋白等指标及彩超对血管斑块探查,分析各指标在糖尿病肾病患者中的诊疗价值。结果与普通2型糖尿病患者相比,糖尿病肾病患者体质指数、糖化血红蛋白、尿素氮、血肌酐、血尿酸、β2微球蛋白、总胆固醇、甘油三酯、低密度脂蛋白胆固醇和尿微量白蛋白均升高,P<0.05,均具有统计学意义,而经治疗后糖尿病肾病患者的尿微量白蛋白下降,P<0.05,具有统计学意义。结论糖尿病肾病的发病是与包括体重、血糖、血脂、蛋白质等多因素代谢紊乱有关,通过规范合理治疗能使病情有所改善。 Objective To observe the changes of some indexes on physiology and biochemistry between simple patients with Type Ⅱ Diabetes Mellitus and the ones with TypeⅡ diabetic nephropathy, we could further understanding the meanings of these indexes on how to diagnose and treat the patients with Type Ⅱ diabetic nephropathy. Methods We summarized some clinical data about above-mentioned patients, such as body mass index(BMI), glycosylated hemoglobin, renal function, blood fat, urinary micro albumin, and the states of vascular plaque, then to analyze some values of these indexes on these patients with Type Ⅱ diabetic nephropathy. Results Compared with simple patients with Type 2 diabetes Mellitus, we find BMI, glyeosylated hemoglobin, blood urea nitrogen, serum ereatinine, blood uric acid, β-2-microglobulin, total cholesterol, triglyeeride, low density lipoprotein cholesterol ( LDL ) , urinary micro albumin of the patients with Type 2 Type Ⅱ diabetic nephropathy were elevatory and the difference is significant in statistics. At the same time, the latter's urinary micro albumin reduced after therapy and the difference was also of significance in statistics. Conclusions The disorder of weight, blood glucose, blood fat, renal function and urinary micro albumin is related with the pathogenesy on Type Ⅱ diabetic nephropathy and these patient 's conditions will get better after normative and reasonable treatment.
