早期足踝主动“环转”运动预防人工关节置换术后下肢深静脉血栓形成的研究

The Research of the Early Active “Circumduction” Movement of Foot and Ankle to Prevent Deep Venous Thrombosis in the Lower Limb after Joint Replacement

探讨早期足踝不同方式的主动运动对预防人工关节置换术后下肢深静脉血栓形成的价值,为预防人工关节置换术后下肢深静脉血栓的形成提供依据。对连续收集的早期进行足踝主动"环转"运动的人工全髋、膝关节置换术的住院患者各100例(以及早期进行足踝主动屈伸运动的人工全髋、膝关节置换术的住院患者各100例)进行下肢深静脉血栓形成发生率的检测;对其进行单因素统计学分析,明确早期足踝主动"环转"运动对预防人工关节置换术后下肢深静脉血栓形成的价值。结果早期进行足踝主动"环转"运动组下肢深静脉血栓形成总发生率为12%,其中人工全髋关节置换术后发生率10%、人工膝关节置换术后发生率14%。早期进行足踝主动屈伸运动组下肢深静脉血栓形成总发生率为23.5%,其中人工全髋关节置换术后发生率21%、人工膝关节置换术后发生率26%。两组间下肢深静脉血栓形成总发生率、人工全髋、膝关节置换术后下肢深静脉血栓形成发生率均具有统计学差异(P<0.05)。说明早期足踝主动"环转"运动可显著降低人工关节置换术后下肢深静脉血栓形成的发生率。

To explore the value of different types of active movements of foot and ankle to prevent deep venous thrombosis in the lower limb after joint replacement in order to provide evidence for prevention of DVT in the lower limb after joint replacement, the incidence of DVT is detected in the lower limb in respectively 100 patients of total hip and knee arthoplasty who took the the early active ＂circumduction＂ movement of foot and ankle and respectively 100 patients of THA and TKA who took the the early active movement of flexion and extension of foot and ankle. Through the univariate statistical analysis , the value of the early active＂ circumduction＂ movement of foot and an- kle to prevent DVT in the lower limb after joint replacement was provided. The incidence of DVT in the lower limb in patients of THA and TKA who took the the early active ＂circumduction＂ movement of foot and ankle was 12%, the THA was 10% ,the TKA was 14%. The incidence of DVT in the lower limb in patients of THA and TKA who took the the early active movement of flexion and extension of foot and ankle was 23.5%, the THA was 21% ,the TKA was 26%. It is resulted that though the univariate statistical analysis, the incidence of DVT in the lower limb was higher in ＂the flexion and extension＂ than in ＂the circumduction＂ （ P 〈 0.05 ）. It is conclused that early active ＂circumduction＂ movement of foot and ankle can significantly reduce the incidence of DVT in the lower limb after joint replacement.
