旋股外侧动脉降支嵌合皮瓣修复舌癌术后缺损 被引量:1

摘要 目的:总结10例应用旋股外侧动脉降支为蒂的穿支嵌合瓣修复舌癌术后软组织缺损的临床效果.方法:应用旋股外侧动脉降支制备一蒂多岛嵌合皮瓣,一期修复10例舌鳞状细胞癌扩大切除术造成的软组织缺损,术后观察舌体形态及功能.结果:10例嵌合瓣全部成活,其中1例术后3d出现血管危象,经处理,皮瓣逐渐恢复正常;3例患者术后1个月接受放疗治疗.随访6~9个月,肿瘤无复发,修复后舌体外形及功能良好.供区术创均直接拉拢缝合,愈合良好.结论:应用旋股外侧动脉降支制备嵌合瓣,利用不同组织进行舌缺损个体化修复,有利于提高修复效果,减少术后并发症. Objective:To observe the effects of the branch-based chimeric flap sourced from descending branch of lateral circumflex femoral artery in restoration of the detects after excision of tongue carcinoma.Methods:10 patients with tongue squamous cell carcinoma underwent the operation of tumor removal and tongue reconstruction with chimeric flaps in one-stage.The form and function of the reconstructed tongue were observed.Results:The flaps in 10 cases all survived.Blood vascular crisis in 1 case occurred due to local over-pressure on the pedicle.After emergency treatment,the flap recovered to normal.Followed up for 6 to 9 months,all patients had no tumor recurrence and the tongue form recovered with satisfactory function.The donor sites were sutured directly and healed well.Conclusion:The chimeic flap based on the primary source of descending branch of lateral circumflex femoral artery has reliable blood supply with appropriate thickness and is optional for three-dimensional restoration of tongue defects after excision of carcinoma.
