注射用鼠神经生长因子联合头穴透刺治疗急性脑出血疗效分析 被引量:6

摘要 目的:观察注射用鼠神经生长因子(NGF)联合头穴透刺治疗急性脑出血的临床疗效,及对患者血清S100B蛋白和神经肽Y(NPY)的影响。方法:急性脑出血患者108例,随机分为常规治疗组27例,头穴透刺组32例及联合治疗组49例。3组患者均予以基本支持治疗;头穴透刺组加用针刺法治疗,联合治疗组患者加用NGF,同时辅以头穴针刺法治疗;疗程为4周。在治疗前后分别用美国国立卫生研究院卒中量表(NIHSS)进行神经功能缺损评分,测定血清S100B蛋白及NPY水平,并分析3组治疗前、后血清S100B蛋白及NPY差值与NIHSS评分差值之间的相关性。结果:联合治疗组治疗总有效率高于常规治疗组(P<0.05);治疗后,3组NIHSS评分、血清S100B蛋白和NPY水平均显著低于同组治疗前(P<0.01);头穴透刺组的NIHSS评分、血清S100B蛋白和NPY水平显著低于常规治疗组(P<0.01),联合治疗组则显著低于其他2组(P<0.01);3组S100B蛋白治疗前后差值与NIHSS评分差值呈正相关(r=0.482,P<0.01);NPY差值与NIHSS评分差值呈正相关(r=0.437,P<0.01);S100B蛋白差值与NPY差值呈正相关(r=0.568,P<0.01)。结论:NGF联合头穴透刺治疗能提高急性脑出血患者的临床疗效,推测可能与其抗脑组织缺血损伤和抗神经元细胞凋亡有关。
