高血压脑出血微创清除术与开颅手术临床效果比较

Comparison of Clinical Effects of Minimally Invasive Operation and Craniotomy for Hypertensive Cerebral Hemorrhage

目的:比较高血压脑出血患者分别实施微创清除术及开颅手术效果差异,探讨微创清除术价值。方法:138高血压脑出血患者作为随机分微创组及常规各69例,常规组实施小骨窗开颅术,微创组根据CT定位,利用立体定向技术,应用YL-1型一次性颅内血肿粉碎穿刺针进行微创治疗。结果:微创组手术时间(47.45±10.34)min、术中出血(15.98±5.12)ml、术后并发症5.80%、治疗总有效91.30%;常规组分别为(78.57±19.35)min、(114.34±18.60)ml、34.69%、92.75%;两组比较治疗效果无显著性差异(P>0.05),但微创组手术时间短、术中出血少、术后并发低(P<0.05)。结论:采用微创的抽吸引流手术对于高血压脑出血的治疗,快速简单,患者具有较高的耐受性,能够较好改善患者的临床神经功能,可广泛应用于各级医院。

Objective：Comparison difference effect of hypertensive cerebral hemorrhage patients were performed minimally invasive surgery and surgical operation,to investigate the value of minimally invasive surgery.Methods：138 hypertensive cerebral hemorrhage patients were randomly divided into minimally invasive group and routine as the 69 cases,The conventional group actualize small bone window craniotomy,the minimally invasive group according to CT positioning,use stereotactic technique, application one-time YL-1 type intracranial hematoma smash puncture actualize minimally invasive treatment.Results：Minimally invasive group operation time （47.45±10.34）min,intraoperative hemorrhage （15.98±5.12）ml,postoperative complications 5.80%,the total effective 91.30%,conventional group were （78.57±19.35）min,（114.34±18.60）ml,34.69%,92.75%;two groups compare the treatment effect had no significant difference （P〉0.05）,but the minimally invasive group shorter operation time,less bleeding,low postoperative complications （P〈0.05）.Conclusion：Minimally invasive aspiration and drainage operation for treatment of hypertensive cerebral hemorrhage,rapid and simple,patients with higher tolerance,can better improve the patient＇s clinical neural function,can be widely used in hospitals of all levels.
