新辅助化学治疗对局部晚期非小细胞肺癌术患者近期疗效分析

Curative Effect Analysis on Neoadjuvant Chemotherapy in Locally Advanced Non-small Cell Lung Cancer

目的：观察分析新辅助化学治疗（化疗）对局部晚期非小细胞肺癌术患者近期临床疗效。方法：筛选我院自2009年1月-2011年12月收治的200例晚期非小细胞肺癌术患者,术前按随机数表法均分为两组,对照组直接采用手术治疗,实验组100例采用术前2周的新辅助化疗后手术；统计记录术中病理分析、手术切除率、术后并发症和近期生存率。结果：实验组根治性切除47例,姑息切除43例,手术切除率为90%,病理性完全切除率为49%,均高于对照组,根治性切除32例,姑息性切除43例,手术切除率为75%,病理性完全切除率为32%,实验组在喉返神经损伤、乳糜胸、肺部感染、胸腔积液、肺心病、呼吸困难和发热、咯血等项目上并发症的发生率均低于对照组,实验组9、15个月生存率分别为69.5%、60.1%,满意度91.6%,均高于对照组59.1%、48.4%,80.6%,实验组复发率为11.6%低于对照组31.2%,组间差异有统计学意义（P〈0.05）。结论：新辅助化疗能有效提高手术切除率,降低术后并发症的发生率,延长近期生存率。

Objective：To observe the analysis of neoadjuvant chemotherapy for locally advanced non-small cell lung cancer patients undergoing recent clinical efficacy.Methods：Screening admitted to our hospital from January 2009 to December 2011,200 patients with advanced non-small cell lung cancer patients undergoing preoperative random number table,were divided into two groups,directly treated by surgery,the experimental group,100 were treated with 2 weeks of preoperative neoadjuvant chemotherapy after surgery;the statistical records intraoperative pathological analysis,surgical resection rate,postoperative complications and short-term survival rate.Results：The experimental group of radical resection in 47 cases,43 cases of palliative resection,surgical resection was 90%,pathological complete resection rate was 49%,was higher than that in the control group,32 cases of radical resection,palliative resection in 43 cases,surgery resection rate was 75%,pathological complete resection rate of 32%,the experimental group,the recurrent laryngeal nerve injury,chylothorax, pulmonary infection,pleural effusion,pulmonary heart disease,difficulty in breathing and fever,hemoptysis projects complications occurred were lower than the control group,the experimental group 9,15 month survival rates were 69.5%,60.1%,91.6% satisfaction,were higher than 59.1%,48.4%,80.6%,the experimental group,the recurrence rate was 11.6% less than 31.2% of the control group,the difference between the groups was statistically significant （P〈0.05）.Conclusion：Neoadjuvant chemotherapy can effectively improve the resection rate,reduce the incidence of post-operative complications,extending recent survival rates.
