联合检测血清CRP、Hcy、尿RBP对MD_2早期肾损害的诊治价值 被引量:10

摘要 目的探讨联合检测血清CRP、Hey、尿RBP对2型糖尿病(T2DM)早期肾损害的诊治价值。方法设糖尿病组(DM,182例)和对照组(60例),糖尿病组分为正常蛋白尿组(UAER〈30mg/24h,62例)、微量蛋白尿组(UAER30~300mg/24h,53例)和大量蛋白尿组(UAER〉300mg/24h,67例)。分别检测血清CRP、Hcy、FBS、BUN、CREA、全血HbAIc和尿RBP水平。结果DM组血清CRP、Hey、FBS、全血HbAlc和尿RBP水平均明显高于对照组(P〈0.05,P〈0.01);CRP、Hcy、尿RBP随着尿蛋白的增多而升高,3组之间的差异具有统计学意义(P〈0.05,P〈0.01),而FBS、伞血HbAlc水平在3组之间则差异不明显(P〉0.05);大量蛋白尿组血清BUN、CREA水平明显高于其他组(P〈0.05),而这两个指标在其他3组之间则变化不明显;血清CRP、Hey、尿RBP联合检测的阳性率明显高于单项检测。结论联合检测血清CRP、Hcy、尿RBP可提高糖尿病肾病(DN)的早期诊断率。 Objective To study the value of joint detection of serum CRP, Hcy and urine RBP for the diagnosis and treatment of the early kidney damage of type 2 diabetes. Methods Setting the diabetes group ( 182 cases ) and control group ( 60 cases) , and 182 patients in the diabetes group were divided into the normal proteinuria group (62 cases ) , the trace proteinuria group (53 cases) and severe proteinuria group (67 cases) according to UAER. The level of CRP, I4cy, FBS, BUN, CREA,HbAle and urine RBP were detected in these groups respectively at the same time. Results The diabetes group showed higher CRP, Hcy, FBS, HbAlc and urine RBP levels than the control group (P 〈0.05, P 〈0.01 ). The levels of CRP, Hey and urine RBP rised as the urinary protein increased in three groups, and the difference had statistical significance among the three groups (P 〈0.05, P 〈0.01 ), while FBS and HbAlc levels had no obvious difference among the three groups(P 〉0.05). The severe proteinuria subgroup showed higher - level BUN and CREA than the other groups ( P 〈 0. 05 ), while the two indexes showed no significant changes among the other three groups. The positive rate of the joint detection of CRP, Hcy and urine RBP levels was distinctly higher than single detection of any other index. Conclusion The combined detection of CRP, Hcy and urine RBP can improve the diagnosis and treatment for the early kidney damage of type 2 diabetes.
