摘要
目的探讨慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者支气管肺泡灌洗液(bronchoalveolar lavage fuid,BALF)中细胞计数和IL-8变化。方法选择本科住院的30例COPD急性发作期患者为A组,稳定期患者30例为B组;健康组8例为C组。对3组分别进行肺功能测定及BALF,BALF处理后作细胞学分析,并检测IL-8水平。结果 COPD组FEV1、FEV1/FVC低于健康组,A组低于B组,差异均有显著性(P<0.01);A、B、C组BALF白细胞总数无显著性差异(P>0.05);而A组BALF中性粒细胞比例和IL-8水平均高于B组,B组高于C组,3组两两比较,差异均有统计学意义(P<0.01);IL-8浓度与肺功能呈负相关(r=-0.792,P<0.01),与PMN呈正相关(r=0.862,P<0.01)。结论 BALF中性粒细胞比例和IL-8水平能反映COPD气道炎症情况。
Objective To observe the changes of cytology and IL - 8 in bronchoalveolar lavage fluid of patients with chronic ob- structive pulmonary disease. Methods Thirty COPD patients with acute exacerbation as group A, 30 COPD cases at remission period as group B, and 8 normal people as group C were involved to determine the pulmonary function, analyze the BALF and detect the levels of IL - 8. Results The FEV1, FEV1/FVC in patients of group A was lower than that in group B and group C, the differences were significant ( P 〈 0.01 ) ; Total number of white blood cells in BALF had no significant difference among the 3 groups (P 〉 0.05 ) ; While neutrophils ratio and IL - 8 levels in BALF of group A were higher than those in group B and group C, there was statistical difference between any two groups (P 〈 0.01 ) ; IL - 8 concentration was negatively correlated with pulmonary function ( r = - 0. 792, P 〈 0.01 ) but positively correlated with PMN ( r = 0. 862, P 〈 0.01 ). Conclusion The IL - 8 level and neutrophil granulocyte ratio in bronchoalveolar lavage fluid (BALF) can reflect airway inflammation of chronic obstructive pulmonary disease.
