摘要
目的用相关性分析糖尿病(DM)患者脑干听觉诱发电位(BAEP与神经传导(NC)间的关系。方法回顾性分析接受BAEP和NC研究的患者,分为DM组43例和对照组43例,其中DM组再次分为4组包括神经病组,非神经组,梗死组和非梗死组,比较各组之间脑干听觉诱发电位峰间潜伏期I~Ⅲ波和I~V波等的差异。结果DM组和对照组之间比较,脑干听觉诱发电位峰间潜伏期I~Ⅲ波和I~V波差异有统计学意义,DM神经组IPLI~III(2.21±0.17)msvs(2.07±0.12)ms(P〈0.01);DM梗死组IPL I~III(2.20士0.17)msvs(2.07±0.12)ms(P〈0.05),DMIPLI~Ⅲ非梗死组(2.20±0.11)msvs(2.07±0.12)ms(P〈0.01);DM神经组IPLI~V(4.16±0.21)msvs(4.02土0.10)ms(P〈0.05)。脑干听觉诱发电位的I~Ⅲ波峰间潜伏期与胫神经运动传导速度有相关性(r=-0.308,P〈0.05)。结论DM患者的I~Ⅲ波峰间潜伏期延长,特别在神经病亚组。这种延长现象可以由听神经损伤解释。DM患者,胫神经,正中神经感觉,腓肠神经神经传导速度与听觉神经病存在相关性。
Objective To study the correlation between brainstem auditory evoked potentials(BAEP) and nerve conduction(NC) in patients with diabetes mellitus(DM). Methods We retrospectively reviewed the results from the subjects who received neurological screening test including BAEP and NC studies:DM group(43 patients) and control group(43 patients). The DM group was subdivided into 4 subgroups including neuropathy, n0n-neuropathy,infarct and non-infarct. The inter-peak latencies (IPL) I-Ill and I-V of the BAEP among different groups were compared. Results IPL I - II and IPL I - V of the BAEP showed statistical significance between the DM and control groups. DM neuropathy subgroup IPL I -III (2.21±0.17) ms vs (2.07±0.12) ms( P 〈0.01) ;DM infarct subgroup IPL I -III (2.20±0.17) ms vs (2.07±0.12) ms( P 〈0.05) ;DM neuropathy subgroup IPL I -V (4.16±0.21) ms vs (4.02±0.10) ms( P 〈0.05). The IPL I -lll showed moderate correlation with tibial motor NC velocity( r =-0. 308, P 0.05). Conclusion Patients with DM have a prolongation in IPL I- III, especially in the neuropathy subgroup. This prolongation in IPL I -Ill would best be explained by acoustic neuropathy. The tihial motor,median sensory,and sural NC velocities correlated with the acoustic neuropathy in patients with DM.
