替吉奥联合奥沙利铂治疗晚期结直肠癌临床研究

Therapeutic effect of Gimeracil and Oteracil Porassium Capsules combined with Oxaliplatin for the treatment of advanced colorectal cancer

目的探讨晚期结直肠癌应用奥沙利铂联合替吉奥临床治疗的效果及其毒副作用。方法选取浙江省德清县第三人民医院2011年6月-2012年6月经病理组织学或细胞学检查证实为晚期结直肠癌患者52例，其中初诊患者20例，复治患者32例；所有患者均应用替吉奥胶囊80mg／（m2·d），2次／d，d1-14，奥沙利铂130mg／m2，d1，21d为1个周期；评价患者疗效及药物不良反应情况。结果①所有患者均完成3个周期的化疗。其中完全缓解2例（3．8％），部分缓解23例（44．2％），稳定18例（34．6％），进展9例（17-3％），总有效率为48．1％（25／52）；总生存时间为5～26个月，1年的生存率为65．4％（34／52）。②主要不良反应包括消化道反应、神经感觉异常、骨髓抑制等。其中消化道反应包括恶心、呕吐21例（40．4％），腹泻23例（44．2％），肝功能损害11例（21．2％），肾功能损害5例（9．6％）；口腔黏膜炎12例（23．1％）；神经感觉异常27例（50．0％）；骨髓抑制包括白细胞降低21例（40．4％），血小板下降14例（26．9％）。患者停药后均在2周内恢复，未影响化疗的正常进行。结论对晚期结直肠癌患者应用奥沙利铂联合替吉奥治疗能有效提高患者近期治疗效果，且毒副作用较低，耐受性好，值得临床应用。

Objective To investigate therapeutic and toxic effect of Gimeracil and Oteracil Porassium Capsules combined with Oxaliplatin for the treatment of advanced coloreetal cancer. Methods 52 patients of advanced colorectal cancer in the Third People＇s Hospital of Deqing County from June 2011 to June 2012 were selected and divided into initial treatment group （20 cases） and retreatment group （32 cases）, all the patients were confirmed as advanced coloreetal cancer by pathologic histology or cytology examination. All the patients were treated with Gimeracil and Oteracil Porassium Capsules 80 mg/（m2.d）, 2 times/d, d1-14, Oxaliplatin 130 mg/m2, dl, 1 cycle was counted for 21 days. Efficacy and adverse drug reactions were evaluated. Results ①All the patients were treated for 3 cycles. 2 cases （3.8%） with CR were found, 23 cases （44.2%） with PR were found, 18 cases （34.6%） with SD were found, 9 cases （17.3%） with PD were found, RR was 48.1% （25/52）; OS were 5-26 months, 1 year survival rate was 65.4% （34/52）. ②Gastrointestinal reaction, abnormal nervous feeling, myelosuppression were the main untoward effect in the treatment. 21 cases （40.4%） with nausea or vomiting, 23 cases （44.2%） with diarrhea, 11 cases （21.2%） with liver damage, 5 cases （9.6%） with renal function damage, 12 cases （23.1%） with oral mucositis were found in gastrointestinal reaction. 27 cases （50.0%） of abnormal nervous feeling were found. 21 cases （40.4%） with white blood cells decline, 14 cases （26.9%） with platelets decline were found in myelosuppression. All the patients were recovered in 2 weeks with no effeeting on the chemotherapy. Conclusion Gimeracil and Oteracil Porassium Capsules combined with Oxaliplatin for the treatment of advanced colorectal cancer can effectively improve the recent treatment effect with low side effects and good tolerance, it is worthy of clinical application.
