自动痔套扎术治疗Ⅱ、Ⅲ期内痔的临床疗效

Clinical Effect of Ruiyun Procedure for Hemorrhoids in the Treatment of Ⅱ or Ⅲ Stage Hemorrhoids

为探讨自动痔套扎术（RPH）治疗Ⅱ、Ⅲ期内痔的疗效，对2011~2012年行RPH治疗的94例Ⅱ、Ⅲ期内痔患者资料进行回顾性分析。结果显示，2011年收治的53例患者单纯行RPH治疗，共使用胶圈168个，术中胶圈脱落6个（3．57％），术后早期（术后2d内）胶固脱落4个（2．38％），总脱落率为5．95％，治愈47例（88．68％）；2012年收治的41例患者行RPH治疗，并于术中在套扎点处注射高渗盐水，共使用胶圈135个，均未发生术中及术后早期胶圈脱落，治愈41例（100％）。随访半年，94例患者术后均未出现感染、肛门狭窄、大便失禁等并发症和后遗症。结果表明，RPH治疗痔操作简单，疗效可靠，患者痛苦轻微，易于接受。套扎点处注射高渗盐水可有效防止胶圈过早脱落，从而提高治疗效果。

In order to investigate the therapeutic effects of Ruiyun procedure for hemorrhoids（RPH） in the treatment of Ⅱ or Ⅲ stage internal hemorrhoids,authors retrospectively analyzed the data of 94 patients with Ⅱ or Ⅲ stage internal hemorrhoids who had received RPH within 2011-2012 yr. As results,among the 53 cases in 2011 yr. only received RPH 47 were cured （88.68%）,the total number of used rubber rings was 168,at intraoperation rubber rings to drop off were 6（3.57% ,6/168）,at early postoperation（within 2 days） rubber rings to drop off were 4（2.38%,4/168）,the total rate to drop off was 5.95% ;among the 41 cases in 2012 yr. who were also injected with hypertonic saline at ligation sites except for receiving RPH, the）, 41 were all cured （100%,41/41）;the total number of used rubber rings was 135 which did drop off neigher at intra- nor post-operation;the follow-up of 0.5yr. did not find the complication,such as,infec- tion,anal stenosis and acathexia, etc and other sequelae in all of 94 cases. Results show that for internal hemorrhoids,RPH has certain curative effect, simple manipulation,and the pain of patients is mild, they like to accept it; besides, injecting hypertonic saline at ligation sites can effectively prevent from rubber rings to drop off at excessive early, thus therapeutic effect is just enhanced.
