斑点追踪成像技术对阻塞性睡眠呼吸暂停综合征患者右心室功能的评价 被引量:1

摘要 目的探讨超声二维斑点追踪成像技术(speckle tracking imaging,STI)评价阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者右心室功能的临床价值。方法选取2012年3月-2013年2月间我院住院或门诊确诊的OSAHS患者61例,正常对照20例,根据睡眠呼吸暂停低通气指数(apnea hypopnea index,AHI)将OSAHS划分为轻、中及重度组,所有研究对象右心室游离壁各节段应变及应变率曲线的分析测量通过STI完成。结果与对照组比较,OSAHS组TAPSE、E/A、IVA减小,RAVM、E/Em、DTm、IVRT增大,差异均有统计学意义(P<0.05),右心室游离壁各节段心肌SR、SRs、SRe、SRe/SRa均减小,SRa增大,差异均有统计学意义(P<0.05)。结论超声斑点追踪成像技术能够评价OSAHS患者右心室功能受损的早期表现。 Objective To study the clinical value of speckle tracking imaging (STI) in assessment of right ventricular function in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods Sixty-one OSAHS patients admitted to our hospital from March 2012 to February 2013 served as an OSAHS group which was further divided into mild OSAHS group, moderate OSAHS group and severe OSAHS group according to their apnea hypopnea index (AHI), and 20 healthy subjects served as a control group in this study. Myocardial strain of right ventricular free wall segments and its curve were analyzed according to their STI. Results The TAPSE, E/A, IVA, SR, SRs, SRe, and SRe/SRa were lower while the RAVM, E/Em, DTm, IVRT, and SRa were higher in OSAHS group than in control group (P 〈 0.05). Conclusion STI can assess the early clinical manifestations of right ventricular function in OSAHS patients.
