直肠癌根治术中双吻合器的应用分析

The Practice of Double Stapling Technique in Radical Resection of Rectal Carcinoma

:[目的]介绍和评价双吻合器在直肠癌低位前切除术中的应用。[方法]对90例直肠癌根治术中行双吻合器吻合的病例进行回顾性分析。[结果]行传统根治术75例 ,扩大根治术7例 ,直肠全系膜切除术(TME)者8例 ,吻合过程顺利。术后切口感染3例(3.33 %) ,吻合口狭窄5例(5.56 %) ,吻合口瘘3例(3.33%)。无手术死亡病例。术后吻合口复发2例 ,腹腔淋巴结转移3例 ,随访期间肝转移5例。全部病例中83例获得随访 ,5年生存率58.6%。[结论]双吻合器应用于直肠癌前切除术中的结直肠吻合具有操作简单 ,节约手术时间和安全等优点 ,并可适当提高低位直肠癌保肛率。

To introduce and evaluate the role of double stapling technique in low anterior resection of rectal cancer.Re_trospective analysis was made on 90 cases of rectal carcinoma with double stapling technique for anastomosis.All cases including 75 cases of traditional radical resection,7 cases of extend radical resection,and 8 cases of total mesorectal excision (TME) were successful and excellent.Complication included anastomotic stenosis in 5 patients (5.56%),anastomotic fistula in 3 patients (3.33%) and wound infection in 3 patients (3.33%).Two patients presented with local recurrence,and 3 cases presented with abdominal cavity nodes metastasis.Liver metastasis was found in 5 cases in the follow-up peroid.Of 90 cases,83 cases had been followed up.The 5-year survival rate was 58.6%.[Conclusion]The double stapling technique is safe,and easily performed for colorectal anastomosis in deep pelvis,and it can enhance the rate of anal-saving operation in low rectal carcinoma.
