携带管路出院患者延续护理服务需求调查分析

Investigation and analysis of transitional care needs among discharged patients with tubes

目的：了解携带管路出院患者的延续护理需求及相关意愿。方法：采用自行设计的问卷对9个省市12家三级医院的1300例住院患者进行调查。结果 ：①与需要专科性管路维护服务的患者相比,需要一般性管路维护服务的患者更愿意选择继续留在医院（χ~2=63.183,P〈0.001）;②携带各种管路患者选择医院护士提供服务的比例高于社区护士（P〈0.001）,选择社区护士作为服务提供者的患者中,专科性管路的患者比例低于一般性管路患者（χ~2=4.203,P=0.040）。结论：应加强和完善延续护理服务中管路维护的规范性,并重点提高社区护士的能力,以满足患者对延续护理服务的需求。

Objective：To explore the transitional care needs of discharged patients with tubes.Methods：Totally 1300discharged patients from 12 Tertiary Grade-A hospitals were investigated by self-designed questionnaire.Results：① The patients with general tube maintenance care needs were more likely to choose hospital-based transitional care service（χ~2=63.183,P＆lt;0.001）; ② The choosing rate of hospital specialized nurse was higher than that of the community nurse（P＆lt;0.001）.Choosing community nurse as service provider among general tube maintenance patients was higher than that among specialized tube maintenance patients（χ~2=4.203,P=0.040）.Conclusions：We should improve the quality of transitional care service for them,strengthen the care ability of community nurses,and satisfy the patients’ needs.
