基于医院延续护理开展现状与问题的质性研究

A qualitative study of the current status and existing problems of hospital-based transitional care

目的:了解医院延续护理服务开展的现状与存在的问题。方法:采用小组访谈法,共访谈护士和护士长17名,访谈资料采用主题框架法进行分析。结果 :通过访谈数据的分析,共得到一级主题8个,二级主题20个。结论:医院在开展延续护理方面仍存在服务规范性较低、服务能力有限和服务形式单一等问题,建议在构建延续护理评估体系、规范护理服务过程、设立延续护理岗位基础上,建立合理的收费机制,并加强与社区的合作,探寻可行的医院-社区护理延续方式。

Objective：To explore the current status and existing problems of hospital-based transitional care.Methods：Totally 17 nurses and head nurses from 9 hospitals were recruited and we adopted semi-structured group interview method and analyzed the data by thematic analysis.Results：Thematic analysis revealed 8 major themes and 20 sub-themes.Conclusions：The problems in hospital-based transitional care practice were unstandardized service process,limited service ability,and simplified service forms.We suggested to construct nursing assessing system,standardize nursing service process,establish transitional care position,set up reasonable charging system,strengthen the cooperation with the community,and explore the feasible hospital-community transitional care mode.
