益气活血方在预防下肢骨折术后深静脉血栓形成的临床疗效

Clinical effect of Yiqi Huoxue Decoction in the prevention of fracture of lower limb deep venous thrombosis

目的探讨益气活血方预防下肢骨折术后深静脉血栓形成的临床疗效。方法 2010年2月至2012年4月,我院收治下肢骨折并行下肢骨折术患者60例,随机分为两组,每组30例患者,对照组给予低分子肝素常规方法预防深静脉血栓,治疗组给予益气活血方,两组患者均连续用药4周,检测血浆活化部分凝血时间(APTT)、凝血原时间(PT),比较D-二聚体(D-dimer)的水平变化,用彩色多普勒超声检查并统计下肢深静脉血栓的发生情况,对比分析两组不良反应发生情况。结果治疗组DVT发生率3.3%,对照组DVT发生率13.3%,两组比较差异有统计学意义(P<0.05);两组用药后PT、APTT均较用药前延长(P<0.05),治疗组用药后PT、APTT变化显著优于对照组,差异有统计学意义(P<0.05);对照组用药后D-dimer水平无明显变化,治疗组用药前后D-dimer水平变化差异有统计学意义(P<0.05),与对照组相比,治疗组用药后D-dimer水平低于对照组,差异有统计学意义(P<0.05);治疗组不良反应发生率0%,对照组不良反应发生率6.7%,两组比较差异有统计学意义(P<0.05)。结论益气活血方预防下肢骨折术后深静脉血栓形成疗效好,副作用少,值得临床推广应用。

Objective To investigate the effects of Yiqi Huoxue Decoction to prevent the clinical curative effect of deep venous thrombosis after lower limb fracture. Methods From 2010 February to 2012 April, 60 cases of patients with fracture of lower limb in parallel in our hospital for lower limb fracture, were randomly divided into two groups, 30 patients in each group, the control group was given the conventional method of low molecular weight heparin to prevent deep venous thrombosis, the treatment group was given Yiqi huoxue fang, two groups of patients were treated for 4 weeks, the serum level of APTT, PT study on the changes, D-dimer, produced by color Doppler ultrasonography and statistics of deep venous thrombosis of lower extremity, comparative analysis the occurrence of adverse reactions of the two groups.Results Treatment group, the incidence of DVT was 3.3%, the control group the incidence of DVT was 13.3%, a significant difference between two groups （P〈0.05）; the two groups after treatment PT, APTT were compared with those before treatment （P〈0.05）, extend the control group after treatment in treatment group was significantly better than PT, APTT change, there was statistically significant difference （P〈0.05）; the control group after treatment D-dimer had no significant changes, changes in D-dimer levels before and after treatment in treatment group had significant difference （P〈0.05）, compared with the control group, D-dimer levels after treatment in treatment group than in the control group, the difference was statistically significant （P〈0.05）; adverse reaction in the treatment group and the control group was 0%, the incidence of adverse reaction was 6.7%, with statistical differences the two group （P〈O.O5）.Conclusion Yiqi Huoxue Decoction to prevent fracture of lower limb deep vein thrombosis of good curative effect, less side effects, is worthy of clinical a^t~lication.
