阴道镜下活检诊断宫颈上皮层内瘤样病变的临床价值分析

Analysis the clinical value of vaginal microscopically biopsy in diagnosis of cervical intraepithelial neoplasm

目的 探讨阴道镜下活检在诊断宫颈上皮内瘤样病变（CIN）的临床价值.方法 对2010年1月～2012年6月在我院行阴道镜下活检诊断为CIN 86例,再行宫颈锥切术进行病理检测,比较两组病检方法.结果 在阴道镜下活检,86例患者诊断为CIN Ⅰ级14例,Ⅱ 级38例,Ⅲ级34例.在宫颈锥切术后病理检测,慢性宫颈炎4例,Ⅰ级12例,Ⅱ 级34例, Ⅲ级31例,其中包括早期浸润癌4例,浸润癌1例.阴道镜下活检结果与宫颈锥切术后病检结果Ⅰ级、Ⅱ 级、Ⅲ级互相比较,差异无统计学意义（P＜0.05）.结论 阴道镜下活检诊断CIN操作简单便捷,对CIN的检出准确率高,可有效防止宫颈癌的发生,值得临床推广应用.

Objective To dicuss the diagnostic value of biopsy colposcopical for cervical intraepithelial neoplasm（CIN）. Methods From January 2010 to June 2012 in our hospital 86 cases diagnosed as CIN under biopsy colposcopica, then taken cervical cone cutting technique for pathological detection, compared inspection methods between the two groups. Results Biopsy after conizations indicated 86 cases diagnosed as CIN,14 cases with CIN I ,38 cases with CIN I1,34 cases with CIN HI, pathological detection after the cervical cone cutting postoperative, chronic cervicitis 4 cases, 12 cases with CIN I, 34 cases with CIN I1,31cases with CIN ]]I,included 4 cases with early invasive carcinoma,1 case with infiltrating carcinoma. The result of biopsy colposcopica and cervical cone cutting postoperative disease inspection results in class I, II level, III level compared with each other, no significant difference, no statistical significance （P〈0.05）. Conclusion Biopsy colposcopica diagnosed cervical intraepithelial neoplasm, the operation is simple and convenient, the CIN detection rate is relatively accurate, can effectively prevent the occurrence of cervical cancer, is worthy to be popularized.
