腹部手术后早期炎性肠梗阻52例临床观察

Clinical observation of 52 cases of early inflammatory ileus after abdominal operation

目的探讨腹部手术后早期炎性肠梗阻的病因、特点及治疗方法。方法收集该院普外科2008年1月至2012年8月收治52例术后早期炎性肠梗阻患者的临床资料,采用中西医结合的方法对患者进行综合治疗,观察临床治愈率及症状缓解时间等。结果 52例患者均经非手术治疗,49例治愈(94.23%),平均治愈时间(12.3±1.7)d;3例因发生机械性肠梗阻、肠绞窄后中转手术,最终治愈。结论早期炎性肠梗阻是腹部手术常见的并发症,一般发生在术后1～3周内,系腹部手术创伤或腹腔内炎症导致肠壁水肿和渗出。绝大多数患者经综合治疗后效果明显,但应密切观察病情,有极少数患者会出现机械性肠梗阻或肠绞窄,须手术治疗方能痊愈。

Objetive To explore etiology, characteristics and therapeutic method of operation early inflammatory ileus after abdominal. Methods Clinical data of 52 cases of early postoperative inflammatory ileus ,who enrolled in general surgery department in the hospital from January 2008 to August 2012 ,were collected. The combination of traditional Chinese medicine and western medicine was applied for comprehensive treatment. The clinical cure rate and relief time of symptoms were observed. Results 52 patients were all with non-surgical treatment,and 49 patients were cured（94.23%） with the average healing time of （12.3＋1.7）d;3 cases were operated due to mechanical obstruction and intestinal strangulation, and got cured ultimately. Conclu- sion The early postoperative inflammatory ileus is a common postoperative complication,which generally occurs 1-3 weeks after surgery,trauma or abdominal surgery department intraperitoneal inflammation leads to intestinal wall edema and exudation. The majority of patients after combined treatment is with obvious effect, but the patients condition should be closely observed;there are very few patients with mechanical obstruction or intestinal strangulation, which is cured after operation.
