朗格汉斯细胞组织细胞增生症1例

One Case of Langerhans Cell Histiocytosis

患者女，1岁4个月。全身出现皮疹并反复发作1年余。均表现为颈前、腹股沟周围散在分布约针尖至小米粒大小圆形肤色丘疹，予外用药可于7～10d消退，也可自行消退，消退后不留痕迹。系统检查未见异常皮损组织病理示：真皮内弥漫性的组织细胞样细胞浸润，浸润的细胞较大，核有异型，呈肾形，不规则形，有切迹；免疫组织化学示：CDla和S-100染色均阳性。诊断为朗格汉斯细胞组织细胞增生症：目前患儿2岁，定期皮肤科及血液科随诊，皮疹反复发作2次，系统检查未见异常，未予特殊处理.

A 16-month-old girl presented with round and fresh-color papules distributing on front of neck and groin. Re- peated episodes of skin rash persisted for more than one year. But it could fade in seven to ten days ＇alter u- sing cxtern＇,dly applied agent,and could fade spontaneously. Once it faded, no trace remained. Her systemic examination was unremarkablc. Histopathological examination showed that diffuse cells like histiocytes infil- Irate in the dermis. Infiltrated cells were bigger. Nuclear was heteromorphy, kidney shape, irregular, not- ched. The immunohistochemical examination indicated positive for CD1 a and S-100. The diagnosis was langc- rhans cell histiocytosis. Until now the lesions of the 2 year＇s old patient also have appeared repeatedly twice b） dermatology amt haematology periodical foUow-up. No symptoms, so we have no special handling.
