左室心内膜起搏在心脏再同步化治疗中的应用研究进展

摘要
心脏再同步化治疗(CRT)通常需要将左室(LV)起搏导线置于冠状静脉分支进行心外膜起搏。常规CRT约有10%的植入失败率和30%的无应答率。LV心内膜起搏技术因其心电传导速度快、激动顺序符合生理、可选择的起搏位置更多等独特优势,在改善心室同步性和血流动力学、减少室性心律失常、提高导线植入成功率等方面表现出良好的应用前景。目前经房间隔途径的起搏导线植入技术最常用。肯定其长期有效性和安全性需要大规模的临床试验证据。预防血栓栓塞、二尖瓣损害等严重不良事件以及研发专用手术工具是推广该技术的关键。
