综合性护理干预对脑出血患者负性情绪、治疗依从性和并发症的影响

Influence of comprehensive nursing intervention on negative emotion, treatment compliance and complication of patients with cerebral hemorrhage

目的 探讨综合性护理干预对脑出血患者负性情绪、治疗依从性和并发症的影响.方法 选择2009年1月～2013年4月68例急性脑出血患者,随机分为护理干预组组和常规护理组.两组患者人院后予以脑出血常规药物对症治疗.常规护理组在此基础上予以常规护理干预,护理干预组在常规护理基础上予以综合性护理干预,干预时间均为4周.观察两组患者治疗前后负性情绪的变化,比较其治疗依从性和并发症的发生率.结果 干预4周后,两组患者汉密顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA）评分[（24.76±3.81）、（20.84±3.12）、（18.91±3.17）、（15.27±2.97）分]均较治疗前[（30.17±4.12）、（26.15±3.52）、（29.87±4.26）、（25.76±3.42）分]明显下降（t=2.21、2.26、2.89、3.02,P＜0.05或P＜0.01）,且护理干预组下降幅度较常规护理组更明显（t=2.32、2.37,P＜0.05）;同时护理干预组的治疗总依从率（88.24％）明显高于常规护理组（61.76％）（x2=6.35,P＜ 0.05）,并发症发生率（14.71％）明显低于常规护理组（41.18％）（x2=5.92,P＜0.05）.结论 综合性护理干预可明显改善急性脑出血的抑郁、焦虑等负性情绪,改善患者的心理状态,提高患者治疗依从性,减少并发症的发生率.

Objective To discuss influence of comprehensive nursing intervention on negative emotion,treatment compliance and complication of patients with cerebral hemorrhage.Methods 68 cases of patients with cerebral hemorrhage from January 2009 to April 2013 were selected and divided into nursing intervention group and routine nursing group at random.The patients in two groups were given symptomatic treatment by routine drugs to treat cerebral hemorrhage.The patients in routine nursing group were given routine nursing intervention for 4 weeks,while the patients in nursing intervention group were additionally given comprehensive nursing intervention for 4 weeks.The changes of negative emotion of patients in two groups before and after medical treatment were observed,and the occurrence rates of treatment compliance and complication were compared.Results After 4 weeks intervention,HAMD and HAMA scores of patients in two groups [（24.76±3.81）,（20.84±3.12）,（18.91±3.17）,（15.27±2.97） scores] were declined obviously than before [（30.17±4.12）,（26.15±3.52）,（29.87±4.26）,（25.76±3.42） scores] （t =2.21,2.26,2.89,3.02,P ＜ 0.05 or P ＜ 0.01）,and the declining rate in nursing intervention group was much higher than that in routine nursing group （t =2.32,2.37,P ＜ 0.05）.Meanwhile,the treatment compliance rate in nursing intervention group （88.24％） was much higher that in routine nursing group （61.76％） （x2=6.35,P ＜ 0.05）,and the occurrence rate of complication （14.71％） was much lower than that in routine nursing group （41.18％） （x2=5.92,P ＜ 0.05）.Conclusion Comprehensive nursing intervention can obviously improve negative emotion like depression,anxiety and so on of patients with symptomatic treatment,improve the mental state,enhance the treatment compliance of patients and reduce the occurrence rate of complication.
