腹腔镜与开腹胆囊切除术的疗效比较

Efficacy comparison of laparoscopic cholecystectomy and open cholecystectomy

目的 比较腹腔镜胆囊切除术与传统开腹胆囊切除术的临床疗效及安全性,为临床工作提供参考.方法 将2009年3月～2012年2月本院收治的156例择期行胆囊切除术的患者随机分成观察组和对照组,其中观察组78例,行腹腔镜下胆囊切除术,对照组78例,行传统开腹胆囊切除术,术中记录患者的一般情况,观察并比较两组患者的疗效及术后并发症发生情况.结果 观察组手术时间、切口长度、术中出血量、住院时间及肠功能恢复时间与对照组比较,差异有统计学意义（P＜0.05）;观察组和对照组切口出血、切口感染、胆漏、胆总管损伤、恶心呕吐及胆囊动脉出血等并发症的总发生率分别为5.1％、26.9％,观察组明显低于对照组,差异有统计学意义（P＜0.05）.结论 腹腔镜胆囊切除术相比传统开腹胆囊切除术具有手术时间短、术中出血量少、患者恢复快、术后并发症发生率低等优点,安全有效,值得在临床推广.

Objective To compare the clinical efficacy and safety of laparoscopic cholecystectomy and open cholecystectomy in order to provide references for clinical work. Methods 156 patients who went eholecystectomy from our hospital from Mar.2009 to Feb. 2012 were randomly divided into observation group and control group with 78 patients in each group.Patients in the observation group were given laparoseopic cholecysteetomy while patients in the control group were given open choleeysteetomy.General condition of the patients were recorded and the efficacy and the oc- currence of postoperative complications of the two groups were observed and compared. Results The operative time,in- cision length,blood loss,hospital stay and intestinal function recovery time of the observation group compared with the control group,statistically significant difference existed （P〈0.05）;The overall incidence of complications as wound bleeding,wound infection,bile leakage,bile duct damage,nausea,vomiting and cystic artery bleeding of the observation group and the control group was 5.1% and 26.9% respectively,the observation group was significantly lower than the control group,statistically significant difference existed （P〈0.05）. Conclusion Compared to conventional open eholeeystectomy,surgery with laparoscopic choleeystectomy has advantages as shorter operative time,less blood loss,quicker recovery and less postoperative complication rate.It is safe and effective and is worthy of clinical application.
